Established by House Bill 2144 (1999), the Advisory Committee on Physician Credentialing Information (ACPCI) develops the uniform applications used by hospitals and health plans to credential and recredential practitioners within the State of Oregon.

Technical Credentialing Questions
Credentialing standards are set and reviewed by The National Committee for Quality Assurance (NCQA) which is a is a private, 501(c)(3) not-for-profit organization dedicated to improving health care quality. Questions about their work should be directed to the NCQA website.
Information
The State of Oregon ACPCI Program is responsible for creating and maintaining a universal credentialing and recredentialing application. Please send completed applications to the health care related organization where you have applied, not to any state agency.
