Each health care facility has 120 days following the close of its fiscal year to file audited financial statements and FR-3s with the Office for Oregon Health Policy and Research.

FR-3 REPORTING FORM
 
For the 2011 reporting year, there are two ways to submit Form FR-3:
 
1.Excel Electronic Form: This is OHPR’s preferred method.
In order to submit your data with this form, download the 2011 Form FR-3 to your desktop.  DO NOT COMPLETE THIS FORM WHILE YOU ARE ON THE WEB.  YOUR DATA WILL NOT BE SAVED!
 
After downloading the document and saving it to your desktop, enter your data directly into the Word form, save, and email the completed form.
 
2. Paper Copy: If you do not have access to Microsoft Excel and /or email capability, your facility’s data can be submitted on a paper form.  In order to submit your data on paper, download the 2011 Paper Form FR-3.  This form is in Adobe pdf format.  If you cannot open the document, you can download the necessary software at no cost by clicking on the following:  Adobe Reader. Please type or print clearly.

COMMUNITY BENEFIT REPORTING
 
Attention Financial Officers and Controllers:
 
Hospitals with a CBR form submission deadline in February, March, or April 2011 will receive an automatic extension to Tuesday, May 31st to submit the revised CBR form.  For all other CBR form deadlines, the regular waiver extension process remains the same.

INPATIENT DISCHARGE DATA
Reporting
 
This information is reported to the Oregon Association of Hospitals and Health Systems (OAHHS).

Instructions on Ordering Inpatient Hospital Discharge Data
 
Hospital inpatient discharge data is available to purchase for research purposes from the Office for Health Policy and Research. 
 
The dataset includes patient demographics, admission and discharge information, characteristics of the treatment provided and nature of each discharge from Oregon hospitals.  In some cases data from two Washington hospitals is part of the public use data file.   The Hospital Discharge Layout 2010 presents the variables included in the current public data set.
 
Annual data files are available in comma-delimited format, accompanied by documentation, file layout, and lookup tables, for calendar years 1990 through the most recent year available – currently 2010.  The cost is $250 per year of data requested
 
Most requests are for one or more calendar years for all discharge records, but custom requests - e.g., specific hospitals or county of residence - can also be arranged as long as they involve simple criteria based on one or more of the publicly-available fields.  Custom requests may require additional fees.  The number of discharge varies by year, generally 350,000 to 400,000 records.
 
 
Prospective purchasers are required to complete the following three steps:
 
 I.    Submit a  Research Data Request (D-1).  Complete the Research Data Request and submit to OHPR with forms D-2 and D-3 (below) as instructed on the form.
 
 II.  Submit  Data Order Form (D-2).  Complete the Data Order Form and submit to OHPR with the Research Data Request (Form D-1), the signed Data Use Agreement(s) (Form D-3) and full payment.
 
 III. Complete the Data Use Agreement (D-3). Researcher and all persons with access to the data are required to read and sign a Data Use Agreement (Form D-3).  Submit with completed forms D-1 and D-2. After the request has been approved and full payment has been received, the data order will be sent out. Address checks to Office for Oregon Health Policy & Research.
