The Family Health Insurance Assistance Program, or FHIAP, helps thousands of Oregon families pay the monthly premium for high-quality, private health insurance plans.
 
If you complete this form, an application will be mailed as funds allow. Only applications for kid's subsidies are being mailed at this time. Adults will be placed on our reservation list. Please indicate your entire family's information.
