Facilitating Licensure

The Oregon Health Licensing Agency (OHLA) created Tools for Schools to facilitate the licensing process for graduating students of Oregon private career schools.  Our goal is to help qualified candidates obtain licensure as quickly as possible so they can start working.

Transcript Submission
OHLA and the Board of Body Art Practitioners accept transcripts approved by the Department of Education, Private Career Schools
 
Transcripts must include the following:
  • Applicant identity information

  • Field of practice(s) enrolled under

  • Specific hour requirements for each field of practice (if applicable)

  • Enrollment information

  • Signature by an authorized representative on file with OHLA (see below).
An authorized representative is anyone listed on the Authorized Representatives for Official Transcripts Form that must be submitted to OHLA by all career schools.
 
Please fax transcripts to 503-370-9004.  Transcripts must be submitted directly to OHLA from the career school.

Agency Closures
OHLA professions
The Oregon Health Licensing Agency is closed on state holidays and mandatory unpaid furlough days through 2012.
