|
Final orders are disciplinary actions that resulted in a final penalty, or settlement, against a licensee. Disciplinary actions result from complaints against licensees by consumers, complaints by other licensed professionals, investigations initiated by the Oregon Health Licensing Agency (OHLA), or from violations found during the course of facility inspections by OHLA inspectors.

Final orders are listed alphabetically by the licensee's last name. This may not be a complete listing of final orders issued by OHLA.
