Social Security Card Required for Examinations

The Oregon Health Licensing Agency (OHLA) requires all applicants for a professional license, certification, or registration to provide a valid Social Security card for walk-in examinations.
 
Applicants unable to provide a Social Security card must provide written authorization or verification from the Social Security Administration. The Social Security Administration office is located at:

Applicants who cannot provide verification in the form of a Social Security card or authorization/verification will not be able to sit for the examination.
