ACPCI develops the uniform applications used by hospitals and health plans to credential and recredential practitioners within the State of Oregon.

The State of Oregon ACPCI program is responsible for creating and maintaining a universal credentialing application. Please send completed applications to the appropriate private credentialing facility. Applications are not accepted by this state agency.
