2009 Credentialing and Recredentialing Update

The Oregon Practitioner Credentialing Application and the Oregon Practitioner Recredentialing Application have been reviewed and amended by the Advisory Committee on Physician Credentialing Information (ACPCI). Both documents have been submitted to the rule-making body and will become effective at a date yet-to-be-determined in 2009.

The State of Oregon ACPCI Program is responsible for creating and maintaining a universal credentialing and recredentialing application. Please do not send applications to this or any other state agency.
