Annual Hospital Discharge Data
Hospital inpatient discharge data is available to purchase for research purposes from the Office for Health Policy and Research. 
 
The dataset includes patient demographics, admission and discharge information, characteristics of the treatment provided and nature of each discharge from Oregon hospitals.  In some cases data from two Washington hospitals is part of the public use data file.   The Hospital Discharge Layout 2007  presents the variables included in the current public data set.
 
Annual data files are available in comma-delimited format, accompanied by documentation, file layout, and lookup tables, for calendar years 1990 through the most recent year available – currently 2007.  The cost is $75 per year of data requested
 
Most requests are for one or more calendar years for all discharge records, but custom requests - e.g., specific hospitals or county of residence - can also be arranged as long as they involve simple criteria based on one or more of the publicly-available fields.  Custom requests may require additional fees.  The number of discharge varies by year, generally 350,000 to 400,000 records.
 
 
Prospective purchasers are required to complete the following three steps:
 
 I.    Submit a  Research Data Request.  Complete the Research Data Request (FormD-1) and submit to OHPR as instructed on the form.
 
 II.  Complete the  Data Order Form.  After the research Data Request (FormD-1) has been approved, complete the Data Order Form (FormD-2) and submit to OHPR with the signed Data Use Agreement(s) (FormD-3) and full payment.
 
 III. Complete the Data Use Agreement . Researcher and all persons with access to the data are required to read and sign a Data Use Agreement (FormD-3).  After the Research Data Request (FormD-1) has been approved,  submit the signed Data Use Agreement(s) with the Data Order Form (FormD-2) and full payment.
 
A CD(s) will be mailed once OHPR receives payments and signed Data Use Agreement.
