 Dear Doctor:
The following documents comprise the application packet for licensure as a medical or osteopathic physician (MD/DO) or podiatric physician (DPM) in the State of Oregon:
- General Information on the licensure process
- Instructions for completing the application form
- Application form
- Forms for verification of training, employment, etc.
READ ALL INSTRUCTIONS - Please read all instructions and the general information on licensure BEFORE completing the application. You must complete all parts of the application form and send it to the Board. Please make a copy of the completed application form for your own records. You may make as many copies of the various verification forms as you need to send to the different sources for verification of state licensure, training, employment, etc.
FEES NON-REFUNDABLE - Please be aware that all FEES submitted are NON-REFUNDABLE AND NON-TRANSFERABLE, and cannot be credited or prorated. It is your responsibility to determine whether or not you meet Oregon's licensure requirements. Fees submitted will not be refunded if it is determined that you are not eligible for licensure.
PROCESSING AN APPLICATION - Applications are processed in date order received, and on average take eight weeks to complete. If you have malpractice or disciplinary history, or derogatory information is received by the Board, it may take three months or longer to complete your application file.
ONLINE STATUS REPORT - As soon as your application and items received are entered on our database, you will receive a letter notifying you of your applicant number with instructions to access the Online Status Report. We ask your cooperation in not contacting the Board office regarding the status of your file until you receive this letter and have viewed your application status on the Online Status Report.
ADDRESS CHANGES - Please keep the Board informed of any changes to your mailing address to ensure receipt of status reports and initial registration paperwork that you must return to the Board prior to licensure.
QUESTIONS? - If you have any questions that are not answered by reading the general information on applying for a license and the application instructions, you may telephone the Board office at the above number from 9 AM – 12 PM and 1 – 3 PM.
| PLEASE NOTE: If you plan to serve a short elective rotation or a fellowship in the state of Oregon for one or two years, you are not required to complete the enclosed application forms, unless your fellowship program requires an unlimited license. However, you are required to obtain a limited license. Please contact this Board to receive instructions and an application to apply for a limited license. |
| If you already have been issued an Oregon license and need to reinstate a lapsed license and/or reactivate a license so you may practice in the state of Oregon, you should contact this office to receive the proper forms to reinstate/reactivate your Oregon license. Do not use this application packet to reactivate your license. |
Teleradiology: A radiologist in another state does not need an Oregon license in order to receive and read radiological images from a hospital or clinic in Oregon, and to transmit an interpretation of these images to a hospital or clinic to be used by the primary care physician in forming a diagnosis for the treatment of his/her patient(s).
TIPS FOR A SMOOTH APPLICATION PROCESS
- Read the instructions, and then completely fill out the application form. Attach a passport quality photo, sign and notarize the application, and send it in with the $427.00 filing fee for MD/DO or the fee of $392.00 for DPM.
- Use the Online Status Report (OSR) to view the current status of your application as frequently as you choose.
- Please do not commit to a practice start date; the Board cannot guarantee that your application will be complete by the time you want to start practicing. Do not put a down payment on a house, for the same reason.
- Request verifications of your medical school, postgraduate training, state licensure, employment, at the same time you fill out the application form. It takes time for these sources to process your request and send the verification to the Oregon Medical Board.
- Select one person to follow the processing of your application. This can be you or a recruiting agency. It is very difficult for Board staff to process your application and those of other applicants if you and two or three other people interested in your application all telephone and inquire about the status of your application. Please be aware that if you have responded "yes" to a personal history question, you may be the only person we can talk to if further information or documentation is needed because of the issue of confidentiality.
