HPP Transfers to DHS
Oregon Medical Board's Health Professionals Program is now closed.

In 2009, the legislature voted to close each health licensing board's Health Professionals Programs and created one program for all the health licensing boards who wish to participate under the Department of Human Services (See Senate Bill 2345). This transition occurred on July 1, 2010.

The Department of Human Services issued a Request for Proposal for a third party to administer the new Health Professionals' Services Programs.  The contract was awarded to Reliant Behavioral Health, LLC and signed in mid-June, 2010. The Oregon Medical Board opted to utilize the new Health Professionals' Services Program for their licensees who have a substance use disorder and/or mental health disorder.

For any licensee who wishes to receive assistance for a substance use disorder and/or mental health disorder on his/her own, or is involved with the program, you may contact the Health Professionals' Services Program at 1-888-802-2843.

The Health Professionals' Services Program will also provide compliance letters to licensing, insurance and credentialing entities upon request accompanied by a valid Consent for the Release of Confidential Information signed by the licensee participating in the program.

For participants of the Oregon Medical Board's Health Professionals Program (HPP) who had closed records, the closed records were scanned onto CD and have been sent to a third party (Scan-It) for storage. These records will be maintained by Scan-It for ten years after the completion date. You must contact Scan-It directly to obtain a Release of Confidential Information form to sign to obtain a copy of your records and Scan-It will charge a fee to make a disc of your record available. If you completed HPP more than 10 years ago, your records were simply destroyed and not sent to Scan-It.

If you only obtained an assessment or evaluation, but were not enrolled in HPP, the records will only be retained for two years to conform with Oregon records retention policy and then destroyed. If your assessment or evaluation was more than two years ago, your records were simply destroyed and not sent to Scan-It. These assessment/evaluation records are not available to anybody.
