Physician Assistant Practice Agreement and Termination Now Online

Practice Agreements are now submitted online. Supervising physicians must log in to the Board’s Online Services using their physician license number and choose Practice Agreements in the lower left of the page, under the Supervising Physicians section. From there, the physician may enter the details of the supervisory relationship, attest to and submit the practice agreement. Then the PA must also log in, review, attest to and submit the practice agreement. This process must be completed within 10 days of the physician assistant beginning practice.
Supervisors and PAs now also report termination of a supervisory relationship online by logging in as described above. The Board must receive notices of termination from the supervisor and from the PA within 15 days following termination of supervision.
