License Verification by Phone
- By calling this number (971-673-2700 8:00a.m. to 12:00p.m. and 1:00p.m. to 4:30p.m. Pacific Time) a person can obtain the following information about any health care provider licensed by the Board: license number, date first licensed in Oregon, expiration date, license status, whether or not a licensee is in good standing with the Board, mailing address, business address, business phone number, year of birth, specialty, and location of patient records. Limited to 3 names per phone call. Cost: No charge.
Malpractice Search
- Malpractice Search: Information reported by insurance companies on closed claims of malpractice made against Board licensees in Oregon. A closed claim is a claim that has been reported as settled, dismissed, or paid. Cost: $10.00 per name.
- Closed Malpractice Cases Report: Monthly report of newly closed malpractice claims made against Board licensees in Oregon, includes licensee name, license number, city and zip code, name of insurer, description of the closed malpractice case, and closure data. Cost: $15.00 per monthly report.
