The Oregon Medical Board offers several ways to obtain verification of licensure in Oregon:
- Anyone can access the on-line License Verification Service to obtain primary source licensure verification, free of charge. To access the License Verification Service, click here.
- Oregon licensees can also utilize VeriDoc to send verification of their Oregon license to other state medical boards electronically for a fee. To access VeriDoc, please visit www.veridoc.org.
- If a printed verification sent directly from the Board is required, a request form along with the appropriate fee must be submitted.
