Preliminary Eligibility Evaluation (PEE) form - Historic District [doc] – Complete and return this document to the SHPO for a prelminary assessment of a potential historic district’s eligibility for the National Register
Multiple Property Documentation Form (MPD) [doc] – For use with a Multiple Property Submission (MPS)
Use the following form to complete National Register nominations for single properties and historic districts. Note: the form is formatted to Oregon SHPO standards, including font size, format, and page numbering.
