Toolkit for Safe Surgery
The Safe Surgical Checklist developed by the World Health Organization is an evidence-based best practice to decrease postoperative complications. The checklist format is an easy-to-follow communication tool listing critical elements that O.R. surgeons and staff need to consider at three separate times: when the patient enters the surgical area; immediately before the incision; and prior to the patient’s transfer to the recovery area. The checklist is an additional safeguard for surgical teams, decreasing risks from distractions, interruptions, and memory slips in busy operating rooms.
The Patient Safety Commission Board has recommended implementation of the checklist and is participating with its partners in the Oregon IHI Network (Acumentra Health, CareOregon, Oregon Association of Hospitals and Health Systems, Oregon Medical Association, Oregon Nurses Association and the Oregon Patient Safety Commission) to assure adoption in every hospital performing surgery in Oregon.
In collaboration with the Oregon IHI Network, the Columbia River Region (Oregon and SW Washington) Chapter of the Association of peri-Operative Registered Nurses (AORN) and the Metro Area Surgical Services Management Organization modified the original WHO checklist and developed the Oregon Safe Surgical Checklist. It includes elements of the Joint Commission-required Universal Protocol, rearranges some of the elements, and adds other important safety aspects, all within the framework of the original checklist, thus maintaining its evidence base.
