Disclosure and Written Notification

Participants in the Oregon Patient Safety Commission's adverse event reporting program agree to provide written notification to patients/families if serious adverse events occur. But how should this disclosure process work? The Commission offers resources and a safe table to improve disclosure practice throughout the state.
