Board of Nursing Disciplinary Actions Taken in April 2013

Benton County

  • The Board accepted the voluntary withdrawal of Donna M. Heavrin’s RN application (087006891RN) for using intoxicants to the extent injurious to herself or others, and failing to answer questions truthfully.  She resides in Blodgett, Ore.

Clackamas County

  • The Board suspended Tammy M. Swan’s Registered Nurse license (083040595RN) for 120 days, for failing to supervise persons to whom nursing tasks have been assigned.  She resides in Oregon City, Ore.

Coos County

  • The Board accepted the voluntary surrender of Robin S. Rasmussen’s Registered Nurse license (001009380RN) for obtaining unauthorized drugs, a Criminal Mistreatment conviction, and failing to conform to the essential standards of acceptable nursing practice.  She resides in North Bend, Ore.

Douglas County

  • The Board accepted the voluntary surrender of Diana M. Young’s Registered Nurse license (099000607RN) for violating the terms and conditions of a Board Order.  She resides in Roseburg, Ore.
  • The Board suspended Stormie D. Page’s nursing assistant certificate (200312382CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Riddle, Ore.
  • The Board placed Lisa C. Shaffer’s Registered Nurse license (200242682RN) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Gardiner, Ore.

Josephine County

  • The Board reprimanded Jeanne B. Zoppo’s Registered Nurse license (089006518RN) for failing to respect client dignity and rights, and failing to communicate information regarding client status to healthcare team.  She resides in Grants Pass, Ore.

Lane County

  • The Board accepted the voluntary surrender of Kristen M. Wassom’s nursing assistant certificate (200910900CNA) for attempting to obtain drugs without authorization, and using intoxicants to the extent injurious to herself or others.  She resides in Springfield, Ore.
  • The Board suspended Tina M. Rose’s practical nurse license (200230091LPN) for 30 days, for inaccurate recordkeeping, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Eugene, Ore.
  • The Board suspended Judy Y. Miller’s Registered Nurse license (000030703RN) for a minimum of 14 days, for failing to maintain tax compliance with the Oregon Department of Revenue (per ORS 305.385).  She resides in Mapleton, Ore.
  • The Board revoked the practical nurse license of Bonnie M. King (089003177LPN), of Eugene, Ore., for practicing without a valid license, and violating the terms and conditions of a Board Order.
  • The Board placed Brittany J. Cumming’s nursing assistant certificate (200510256CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Springfield, Ore.

Linn County

  • The Board accepted the voluntary surrender of Wanda L. Kahl’s Registered Nurse license (200340755RN) for failing to take action to preserve client safety, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Albany, Ore.
  • The Board reprimanded Donald A. Williamson’s Registered Nurse license (091000215RN) for deceit in the practice of nursing.  He resides in Albany, Ore.
  • The Board reprimanded Michelle S. Nilsen’s Registered Nurse license (200241527RN) for performing acts beyond her authorized scope, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Harrisburg, Ore.

Marion County

  • The Board accepted the voluntary surrender of Danielle M. Sylvia’s practical nurse license (201030521LPN) due to convictions for Disorderly Conduct, Initiating a False Report, and Menacing, and failing to cooperate with an investigation.  She resides in Keizer, Ore.
  • The Board accepted the voluntary surrender of Loretta L. Etherton’s Registered Nurse license (094000216RN) for violating the terms and conditions of a Board Order.  She resides in Salem, Ore.
  • The Board accepted the voluntary surrender of Sanjuana J. Diaz’s nursing assistant certificate (200910228CNA) for exploiting the client relationship for personal gain, failing to competently perform CNA duties, and failing to cooperate with the Board during an investigation.  She resides in Salem, Ore.

Multnomah County

  • The Board accepted the voluntary surrender of Kevin C. Healy’s Registered Nurse license (084059404RN) for using intoxicants to the extent injurious to himself or others.  He resides in Portland, Ore.
  • The Board accepted the voluntary surrender of Denise N. Marker’s nursing assistant certificate (201211725CNA) for violating the terms of a Board Order.  She resides in Fairview, Ore.
  • The Board suspended Al E. Kuschner’s Registered Nurse license (083025558RN) for 60 days, for violating the terms and conditions of a Board Order, failing to maintain professional boundaries with a client, and failing to conform to the essential standards of acceptable nursing practice.  He resides in Portland, Ore.
  • The Board suspended Martha E. Perez’s nursing assistant certificate (200911181CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Portland, Ore.
  • The Board suspended Ronald L. Clark’s practical nurse license (201130016LPN) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  He resides in Gresham, Ore.
  • The Board reprimanded Ruby D. Tanner’s practical nurse license (000005175LPN) for incomplete and inaccurate recordkeeping, failing to administer medications, and failing to communicate information regarding client status to healthcare team.  She resides in Portland, Ore.
  • The Board reprimanded Joshua K. Henson’s Registered Nurse license (200842940RN) for using intoxicants to the extent injurious to himself or others.  He resides in Portland, Ore.

Umatilla County

  • The Board placed Cherita M. Waters’s nursing assistant certificate (000043766CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Pendleton, Ore.

Wasco County

  • The Board accepted the voluntary surrender of Luis M. Preciado’s nursing assistant certificate (200712630CNA) for exploiting the client relationship for personal gain.  He resides in The Dalles, Ore.

Washington County

  • The Board accepted the voluntary surrender of Randi L. Bendix’s Registered Nurse license (201040907RN) for obtaining drugs from the workplace without authorization, inaccurate recordkeeping, and using intoxicants to the extent injurious to herself or others.  She resides in Tualatin, Ore.
  • The Board suspended Mark W. Rapant’s nursing assistant and medication aide certificates (000033324CNA/200820043CMA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  He resides in Hillsboro, Ore.
  • The Board suspended Maura A. Banse-Fay’s Registered Nurse license (095006629RN) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Tualatin, Ore.
  • The Board reprimanded Michael R. Tankus’s Registered Nurse license (200942857RN) with conditions for failing to maintain client records in a timely manner.  He resides in Gaston, Ore.
  • The Board placed Lewis D. Panzer’s Registered Nurse license (200841756RN) on probation for 12-months for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  He resides in Hillsboro, Ore.
  • The Board denied the CNA application of Sara Hastings (200912117CNA) for failing to answer questions truthfully, and using intoxicants to the extent injurious to herself or others.  She resides in Beaverton, Ore.
    Yamhill County
  • The Board placed Kristin R. Snyder’s Registered Nurse license (094006037RN) on probation for 36-months for using intoxicants to the extent injurious to herself or others.  She resides in Newberg, Ore.

Out of State

  • California—The Board accepted the voluntary surrender of Ruth A. Thomas’s Family Nurse Practitioner certificate (200950165NP) for violating the terms and conditions of a Board Order.  She resides in Modesto, Calif.
  • Washington—The Board revoked the Registered Nurse license of Leona M. Smith (097006703RN), of Vancouver, Wash., for failing to respect client rights and dignity, violating client rights to privacy, failing to answer questions truthfully, and failing to cooperate with the Board during the course of an investigation.
