Board of Nursing Disciplinary Actions Taken in April 2014

Benton County
  • The Board accepted the voluntary surrender of Kathleen N. Johnson’s Registered Nurse license (200241496RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Corvallis, Ore.

Clackamas County

  • The Board accepted the voluntary surrender of Timothy R. Hughes’s nursing assistant certificate (201210783CNA) for violating the terms and conditions of a Board Order.  He resides in Portland, Ore.

Coos County

  • The Board revoked the Registered Nurse license of Wendi A. Schaefer (091007383RN), of Coos Bay, Ore., for failing to implement the plan of care, failing to maintain professional boundaries with a client, inaccurate and incomplete recordkeeping, and failing to conform to the essential standards of acceptable nursing practice.
  • The Board accepted the voluntary surrender of Mary E. Forsberg’s Registered Nurse license (000029268RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Coquille, Ore.

Deschutes County

  • The Board suspended Heather L. Shaw’s Registered Nurse license (200942469RN) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Bend, Ore.

Douglas County

  • The Board placed Renae C. Green’s nursing assistant certificate (200611824CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Sutherlin, Ore.
    The Board placed Susan A. Edwards’s Registered Nurse license (079011403RN) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Roseburg, Ore.

Jackson County

  • The Board revoked the nursing assistant certificate of Jocelyn V. Perez (201112798CNA), of Medford, Ore., for failing respect client rights, exploiting the client relationship for personal gain, and failing to cooperate with the Board during the course of an investigation.
  • The Board accepted the voluntary surrender of Carrie C. Meier’s nursing assistant certificate (200510836CNA) for violating the terms and conditions of a Board Order.  She resides in Medford, Ore.

Lane County

  • The Board denied the CNA application of Nicole T. Straney, of Eugene, Ore., for failing to answer questions truthfully and engaging in behavior unbecoming to a nursing assistant.
  • The Board accepted the voluntary surrender of Arthur A. Renda’s practical nurse license (201130537LPN) for conduct derogatory to the standards of nursing.  He resides in Cheshire, Ore.
  • The Board reprimanded Ruthanne Olson’s Registered Nurse license (200341543RN) with conditions for failing to respect client dignity, failing to maintain professional boundaries with a client, and client abuse.  She resides in Westlake, Ore.
  • The Board revoked the practical nurse license of Bettina F. Madrid (200830457LPN), of Coburg, Ore., for client neglect, attempting to obtain unauthorized drugs, falsifying client records, and failing to conform to the essential standards of acceptable nursing practice.
  • The Board placed Michael G. Hinton’s practical nurse license (201030301LPN) on probation for 24-months for using intoxicants to the extent injurious to himself or others, and a domestic violence conviction.  He resides in Eugene, Ore.
  • The Board placed Leslie Celeste’s nursing assistant certificate (200811428CNA) on probation for 12-months for client neglect, failing to respect client rights and dignity, and violating rights of privacy.  She resides in Eugene, Ore.

Marion County

  • The Board placed Emily T. Pantalone’s nursing assistant certificate (200913383CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Salem, Ore.
  • The Board reprimanded Janine K. Nelson’s nursing assistant and medication aide certificates (000031252CNA/000031252CMA) for inaccurate and incomplete recordkeeping, and failing to answer questions truthfully.  She resides in Salem, Ore.

Multnomah County

  • The Board reprimanded Gloria L. Stevenson’s practical nurse license (201230222LPN) with conditions for inaccurate recordkeeping, falsifying a client record, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Portland, Ore.
  • The Board reprimanded Ablavi V. Langlidja’s nursing assistant certificate (200611655CNA) for failing to competently perform the duties of a nursing assistant.  She resides in Portland, Ore.
  • The Board revoked the nursing assistant certificate of Jalene R. Dick (200913468CNA), of Gresham, Ore., for client neglect, failing to respect client rights, and exploiting the client relationship for personal gain.

Union County

  • The Board accepted the voluntary surrender of Lisa L. Kennon’s nursing assistant certificate (000041314CNA) for representing herself as a CNA without current CNA certification.  She resides in La Grande, Ore.
  • The Board accepted the voluntary surrender of Tena L. Ferguson’s nursing assistant certificate (200712721CNA) for failing to meet employment requirements for renewal of her certification.  She resides in La Grande, Ore.

Washington County

  • The Board reprimanded and placed a 15-day suspension on Tina L. Imdieke’s Registered Nurse license (200840759RN) for failing to maintain client records in a timely manner, and incomplete recordkeeping.  She resides in Portland, Ore.
  • The Board reprimanded Ritah Giesbers’s practical nurse license (201130461LPN) with conditions for failing to communicate information regarding client status, incomplete recordkeeping, and failing to take action to preserve client safety.  She resides in Sherwood, Ore.
  • The Board accepted the voluntary surrender of Kathy M. Florian’s nursing assistant certificate (201400980CNA) for  violating the terms and conditions of a Board Order.  She resides in Forest Grove, Ore.
  • The Board accepted the voluntary surrender of Carey A. Christian’s Registered Nurse license (200841429RN) for attempting to obtain unauthorized drugs, and violating the terms and conditions of a Board Order.  She resides in Tigard, Ore.
  • The Board placed Jennifer A. Barren’s Registered Nurse license (200941762RN) on probation for 12-months for failing to maintain professional boundaries with a client, and incomplete recordkeeping.  She resides in Portland, Ore.
  • The Board accepted the voluntary surrender of Nenice M. Andrew’s Registered Nurse license and Adult Nurse Practitioner certificate (079043195RN/079043195N3) for conviction for Tampering with Drug Records, prescribing drugs for personal use, and obtaining unauthorized controlled drugs.  She resides in Hillsboro, Ore.

Yamhill County

  • The Board suspended Victoria M. Tooley’s nursing assistant certificate (000028872CNA) for 15 days for failing to answer questions truthfully.  She resides in Carlton, Ore.
  • The Board accepted the voluntary surrender of Debra L. Schwartzkopff’s Registered Nurse limited license (L201400639RN) due to a mental health condition that prevents her from practicing nursing, and violating the terms and conditions of a Board Order.  She resides in McMinnville, Ore.

Out of State

  • Washington—The Board accepted the voluntary surrender of Lucy Bentea’s practical nurse license (201030416LPN) for using intoxicants to the extent injurious to herself or others, and violating the terms and conditions of a Board Order.  She resides in Vancouver, Wash.
  • Washington—The Board suspended Melissa R. Cummings’s Registered Nurse license (200642261RN) for 30 days, to be followed by 24-month probation, for using intoxicants to an extent injurious to herself or others, and failing to answer questions truthfully.
