Board of Nursing Disciplinary Actions Taken in April 2017
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 35 nurses or nursing assistants during its April 12, 2017, Board Meeting.

Clackamas County
  • The Board placed the registered nurse license of Devin J. Landry (201393772RN) on probation for 12 months for possessing an unauthorized drug.  She resides in West Linn, Ore.
  • The Board placed the nursing assistant certificate of James E. Cox (201702594CNA) on probation for 12 months for using intoxicants to an extent or in a manner dangerous to himself or others.  He resides in Beavercreek, Ore.
  • The Board placed the nursing assistant certificate of Brianna E. Rowney (201702717CNA) on probation for 24 months for using intoxicants to an extent or in a manner dangerous to herself or others.  She resides in Canby, Ore.
Coos County
  • The Board placed the registered nurse license of Lorraine L. Lytle (201500716RN) on probation for 24 months for using intoxicants to the extent injurious or in a manner dangerous to herself or others.  She resides in Coos Bay, Ore.
Deschutes County
  • The Board placed the registered nurse license of Amanda G. Greening (200942136RN) on probation for 24 months for failing to maintain professional boundaries with a client, and unauthorized removal of narcotics from a person.  She resides in Bend, Ore.
Douglas County
  • The Board reprimanded the nursing assistant certificate of Crystal L. Stroisch (201502153CNA), of Roseburg, Ore., for failing to implement the plan of care developed by the RN, and jeopardizing a patient’s safety.
Jackson County
  • The Board revoked the nursing assistant certificate of Cindy A. Anderson (000015944CNA), of Central Point, Ore., for violating the terms and conditions of a Board Order, and failing to cooperate during the course of an investigation.
  • The Board placed the family nurse practitioner certificate of Kathleen S. Finley (000029650N1) on probation for 12 months for prescribing drugs in an unsafe or unlawful manner, failing to properly assess and document client assessment when prescribing drugs, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Central Point, Ore.
  • The Board revoked the nursing assistant certificate of Misty D. Holt (201211940CNA), of Medford, Ore., for engaging in violent behavior towards a coworker and failing to cooperate with the Board during the course of an investigation.
  • The Board accepted the Voluntary Surrender of Tracy L. Meister’s practical nurse license (201391016LPN) for incomplete and inaccurate recordkeeping, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Newport, Ore.
  • The Board suspended the nursing assistant certificate of Brenda G. Politron (201508343CNA) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation.  She resides in White City, Ore.
Josephine County
  • The Board denied the CNA application of Ashley N. Araque, of Grants Pass, Ore., for demonstrated incidents of neglectful behavior and a conviction for Child Neglect.

Lane County

  • The Board suspended the registered nurse license of Jandyra M. Dubofsky (201041857RN) for one year, to be followed by two-year probation, for inaccurate and incomplete recordkeeping, failing to report actual or suspected incidents of elder abuse, client neglect, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Eugene, Ore.
  • The Board accepted the Voluntary Surrender of Rachel M. Watkins’s nursing assistant certificate (201501153CNA) for the unauthorized removal or attempted removal of money from a person.  She resides in Eugene, Ore.
Linn County
  • The Board reprimanded the registered nurse license and psychiatric mental health nurse practitioner certificate of Marykay H. Alexander (082010334RN/082010334N6), of Lyons, Ore., for implementing standards of care that jeopardize patient safety and failing to conform to the essential standards of acceptable nursing practice.
Marion County
  • The Board revoked the registered nurse license of Jill A. Henschel (089003340RN), of Woodburn, Ore., for failing to comply with the terms and conditions of a Board Order.
  • The Board accepted the Voluntary Surrender of Kuno Jipping’s nursing assistant certificate (200812146CNA) for abusing a person and failing to respect the client’s dignity.  He resides in Salem, Ore.
Multnomah County
  • The Board accepted the Voluntary Surrender of Karen B. Collacott’s registered nurse license (201340832RN) for practicing nursing while impaired, and using intoxicants to the extent injurious to herself or others.  She resides in Portland, Ore.
  • The Board suspended the nursing assistant and medication aide certificates of Randi L. Salinas (200912432CNA/201020094CMA) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation.  She resides in Portland, Ore.
  • The Board accepted the Voluntary Withdrawal of Mark A. Stevens’s CNA application for using intoxicants to an extent or in a manner injurious to himself or others.  He resides in Portland, Ore.
Polk County
  • The Board placed the registered nurse license of Laura J. Guffey (095000531RN) on probation for 24 months for removing unauthorized drugs from the workplace, falsifying an agency record, and using intoxicants to the extent injurious or in a manner dangerous to herself or others.  She resides in Dallas, Ore.
  • The Board placed the registered nurse license of Kerry Kingsley-Smith (200441951RN) on probation for 24 months for removing unauthorized drugs from the workplace and conduct derogatory to the standards of nursing.  He resides in Salem, Ore.
Sherman County
  • The Board reprimanded the registered nurse license and family nurse practitioner certificate of Mark Corey (093007080RN/093007080N1), of Moro, Ore., for failing to take action to promote client safety, and failing to conform to the essential standards of acceptable nursing practice.
Umatilla County
  • The Board accepted the Voluntary Withdrawal of Diane C. Cort-Wagner’s RN and FNP applications (085075504RN/085075504N1) for conduct derogatory to the standards of nursing.  She resides in Pendleton, Ore.
Wasco County
  • The Board placed the registered nurse license of Craig W. Revels (094000411RN) on probation for 24 months due to a conviction for theft of controlled substances from an individual’s home.  He resides in The Dalles, Ore.
Washington County
  • The Board placed the nursing assistant certificate of Ashley N. Egeland (201702593CNA) on probation for 12 months for using intoxicants to an extent or in a manner dangerous to herself or others.  She resides in Tigard, Ore.
Yamhill County
  • The Board denied the RN application of Meredith J. Abdi, of McMinnville, Ore., for discipline taken in Arizona.
  • The Board accepted the Voluntary Surrender of Lisa E. Holladay’s registered nurse license (084056631RN) for violating the terms and conditions of a Board Order.  She resides in McMinnville, Ore.
Out of State
  • Alabama—The Board reprimanded the registered nurse license of Kylie Vinsant (201603280RN), of Sterrett, Ala., for failing to respect the dignity and rights of clients, and violating the client’s right to privacy.
  • California— The Board accepted the Voluntary Withdrawal of Veronica N. James’s RN and FNP applications for failing to conform to the essential standards of acceptable nursing practice.  She resides in Moreno Valley, Calif.
  • Idaho—The Board accepted the Voluntary Surrender of Aubrey J. Comstock’s registered nurse license (201609335RN) for obtaining unauthorized drugs from the workplace and using intoxicants to the extent injurious to herself or others.  She resides in Boise, Idaho.
  • Nevada—The Board accepted the Voluntary Withdrawal of Cameron Swain’s RN application for using intoxicants to an extent or in a manner injurious to himself or others.  He resides in Verdi, Nev.
  • Washington—The Board accepted the Voluntary Surrender of Lynn M. Counts’s registered nurse license (096007077RN) for violating the terms and conditions of a Board Order.  She resides in Goldendale, Wash.
  • Washington—The Board placed the registered nurse license of Jennifer A. Green (092000557RN) on probation for 24 months for using intoxicants to the extent injurious or in a manner dangerous to herself or others.  She resides in Yacolt, Wash.
  • Washington—The Board suspended the nursing assistant certificate of Shelby R. Waddell (201500609CNA) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation.  She resides in Connell, Wash.
