Board of Nursing Disciplinary Actions Taken in August 2012

Clackamas County
  • The Board accepted the voluntary surrender of Steven L. Parker’s Practical Nurse license (000010272LPN) for practicing nursing when unable to perform due to a physical impairment.  He resides in Sandy, Ore.
  • The Board revoked the Registered Nurse license of Robert S. Yater (095006386RN), of Happy Valley, Ore., for violating the terms and conditions of the Health Professionals’ Services Program.
  • The Board reprimanded Teresa Ann Uhl’s Registered Nurse license (092000569RN) with conditions for inaccurate and incomplete recordkeeping, directing another person to falsify records, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Oregon City, Ore.
  • The Board placed Rebecca R. Smith’s nursing assistant certificate (201011778CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Sandy, Ore.
Clatsop County
  • The Board accepted the voluntary surrender of Angela D. Burnell’s nursing assistant certificate (201111468CNA) for violating the terms and conditions of a Board Order.  She resides in Warrenton, Ore.
Coos County
  • The Board accepted the voluntary surrender of Rhonda J. Power’s Registered Nurse license (200040852RN) for violating the terms and conditions of the Health Professionals’ Services Program.  She resides in Coos Bay, Ore.
  • The Board suspended Patrick R. Bohannon’s nursing assistant certificate (201111926CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  He resides in Bandon, Ore.
Deschutes County
  • The Board accepted the voluntary surrender of Gerald A. Wallace’s Registered Nurse license and Certified Registered Nurse Anesthetist certificate (200440309RN/200460013CRNA) for conduct derogatory to nursing.  He resides in Redmond, Ore.
Douglas County
  • The Board accepted the voluntary surrender of Sandra L. King’s nursing assistant certificate (200912277CNA) for violating the terms and conditions of a Board Order.  She resides in Winston, Ore.
  • The Board accepted the voluntary surrender of Fern M. Carstensen’s Practical Nurse license (000004532LPN) for inaccurate and incomplete recordkeeping, failing to take action to promote client safety, and failing to communicate information regarding client status.  She resides in Glide, Ore.
  • The Board accepted the voluntary surrender of Nicole M. Brown’s nursing assistant certificate (200911665CNA) for using intoxicants to the extent injurious to herself or others.  She resides in Myrtle Creek, Ore.
Jackson County
  • The Board accepted the voluntary surrender of Jennifer L. Gervais’ Registered Nurse license (200743839RN) for violating the terms and conditions of the Health Professionals’ Services Program.  She resides in Ashland, Ore.
Josephine County
  • The Board reprimanded Kamala S. Thibodeau’s nursing assistant certificate (000042256CNA) for exploiting the client relationship for personal gain.  She resides in Merlin, Ore.
Lane County
  • The Board suspended Jessica Hallock’s nursing assistant certificate (200911630CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  She resides in Florence, Ore.
  • The Board suspended Leo J. Burns’ nursing assistant certificate (200110755CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  He resides in Florence, Ore.
  • The Board revoked the Registered Nurse license of Lynne G. Carter (095003166RN), of Springfield, Ore., for failing to comply with the terms and conditions of a Board Order.
  • The Board revoked the Registered Nurse license of Janis Giroux (099000676RN), of Eugene, Ore., for failing to maintain client records, failing to conform to the essential standards of acceptable nursing practice, and violating the terms and conditions of the Health Professionals’ Services Program.
Linn County
  • The Board suspended Patrick C. Hash’s nursing assistant certificate (201011958CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  He resides in Albany, Ore.
Marion County
  • The Board placed Ruth A. Thomas’ Family Nurse Practitioner certificate (200950165NP) on probation for 12-months for failing to document properly client assessment when prescribing, and failing to take action to promote client safety.  She resides in Salem, Ore.
Multnomah County
  • The Board revoked the nursing assistant certificate of Jonathan B. Roberson (200712513CNA), of Portland, Ore., for failing to violating the terms and conditions of a Board Order.
  • The Board reprimanded Ola Taiwo’s nursing assistant certificate (200810382CNA) for engaging in unacceptable behavior or verbal abuse toward a client.  She resides in Portland, Ore.
Umatilla County
  • The Board suspended Aaron T. Brown’s nursing assistant certificate (200611711CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  He resides in Stanfield, Ore.
Yamhill County
  • The Board suspended Alana L. Lenahan’s nursing assistant certificate (000039760CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  She resides in Newberg, Ore.
Out of State
  • Washington—The Board reprimanded Sherrie L. Poujade’s Registered Nurse license (094006426RN) for conduct derogatory to the standards of nursing.  She resides in Vancouver, Wash.
  • Washington—The Board accepted the voluntary surrender of Marcy C. Harris’ Registered Nurse license (200440336RN) for violating the terms and conditions of a Board Order.  She resides in Vancouver, Wash.
  • Alaska—The Board suspended Kevin E. Hornby’s Registered Nurse license (201043257RN) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  He resides in Anchorage, Alaska.
  • New Mexico—The Board accepted the voluntary surrender of Michel D. King’s Registered Nurse license (200541150RN) for practicing nursing while impaired and using intoxicants to the extent injurious to herself or others.  She resides in Albuquerque, N.M.
