Board of Nursing Disciplinary Actions Taken in August 2013
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 19 nurses or nursing assistants during its August 21, 2013, Board Meeting.

Jackson County
  • The Board accepted the voluntary surrender of Tiffany P. Smith’s Registered Nurse license (201041317RN) for inaccurate and incomplete recordkeeping, practicing nursing while impaired, failing to answer questions truthfully, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Medford, Ore.
  • The Board suspended Stephen M. Presley’s nursing assistant certificate (000021777CNA) for 14 days with conditions for failing to communicate information regarding client status to his supervisor.  He resides in Medford, Ore.
  • The Board placed Jennifer L. MacDonald’s Registered Nurse license (201040699RN) on probation for 24-months for unauthorized removal of narcotics from the workplace, falsifying a client record, and failing to dispense medications according to law.  She resides in Jacksonville, Ore.
Lane County
  • The Board accepted the voluntary surrender of Elbridge Gerry J. Louthan’s nursing assistant certificate (201112980CNA) for using intoxicants to the extent injurious to himself or others.  He resides in Eugene, Ore.
  • The Board accepted the voluntary surrender of Kimberly S. Hanson’s Registered Nurse license and Family Nurse Practitioner certificate (200440723RN/200450044NP) for failing to maintain professional boundaries with a client, prescribing drugs to an individual who is not her client, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Eugene, Ore.
  • The Board suspended Dustin S. Earls’ nursing assistant certificate (201112398CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  He resides in Vida, Ore.
Marion County
  • The Board placed Milton M. Garcia’s nursing assistant certificate (000042290CNA) on probation for 24-months for using intoxicants to the extent injurious to himself or others.  He resides in Salem, Ore.
Multnomah County
  • The Board accepted the voluntary surrender of Robert V. Geiger’s practical nurse license (201130357LPN) for using intoxicants to the extent injurious to himself or others.  He resides in Portland, Ore.
  • The Board suspended Eunice E. Taylor’s Registered Nurse license (200342095RN) for 60 days for inaccurate and incomplete recordkeeping, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Portland, Ore.
  • The Board revoked the Registered Nurse license of Ginger D. Cross (200141175RN), of Troutdale, Ore., for failing to take action to promote client safety, failing to respect client dignity, client abuse, unauthorized removal of narcotics from the workplace, and practicing nursing while impaired.
  • The Board reprimanded Sarah A. Kuehl’s Registered Nurse license (201242182RN) for a DUII conviction and using intoxicants to the extent injurious to herself or others.  She resides in Fairview, Ore.
  • The Board placed Jason R. Vargo’s Registered Nurse license (200242641RN) on probation for 24-months for inaccurate and incomplete recordkeeping, failing to dispense medications according to law, and using intoxicants to the extent injurious to himself or others.  He resides in Portland, Ore.
  • The Board placed Lucy Bentea’s practical nurse license (201030416LPN) on probation for 24-months for using intoxicants to the extent injurious to herself or others and practicing nursing while impaired.  She resides in Portland, Ore.
Umatilla County
  • The Board placed Teresa A. Hollibaugh’s nursing assistant certificate and Registered Nurse license (200111020CNA/200743567RN) on indefinite probation for practicing nursing when unable to perform due to a physical impairment.  She resides in Pendleton, Ore.
Wasco County
  • The Board accepted the voluntary surrender of Julie A. Vanvalkenburgh’s nursing assistant certificate (000014748CNA) for inaccurate and incomplete recordkeeping, falsifying a client record, and client neglect.  She resides in The Dalles, Ore.
Washington County
  • The Board accepted the voluntary surrender of Deborah L. Kamiwayoshi’s Registered Nurse license (086000228RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Hillsboro, Ore.
  • The Board accepted the voluntary surrender of Jason L. Shorts’ nursing assistant and medication aide certificates (000040053CNA/000040053CMA) for conviction of a crime that bears demonstrable relationship to the duties of a CNA.  He resides in Tualatin, Ore.
  • The Board reprimanded Patricia J. O’Dea’s Registered Nurse license (077038621RN) with conditions for performing acts beyond her authorized scope of practice.  She resides in Tigard, Ore.
Yamhill County
  • The Board reprimanded Nancy A. Macy’s Nurse Midwife Nurse Practitioner certificate (200150033NP) for failing to report suspected incidents of client abuse through the proper channels.  She resides in Newberg, Ore.
