Board of Nursing Disciplinary Actions Taken in August 2014
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 18 nurses or nursing assistants during its August 13, 2014, Board Meeting.

Benton County
  • The Board accepted the Voluntary Surrender of Suzanne J. Brown’s Registered Nurse license (080045381RN) for failing to comply with the terms and conditions of a Board Order.  She resides in Corvallis, Ore.
Clackamas County
  • The Board suspended Jill J. Hearing’s Registered Nurse license (080044705RN) for a minimum of 14-days for failing to cooperate with the Board during an investigation.  She resides in Milwaukie, Ore.
  • The Board suspended Kyra S. Munker’s nursing assistant and medication aide certificates (200510695CNA/201120057CMA) for a minimum of 14-days for failing to cooperate with the Board during an investigation.  She resides in Oregon City, Ore.
  • The Board revoked the nursing assistant certificate of Mary L. M. Sule (201010488CNA), of Milwaukie, Ore., exploiting the client relationship for personal gain.
Deschutes County
  • The Board reprimanded Susan L. Brewer’s Registered Nurse license (201390731RN) for failing to implement the plan of care and failing to conform to the essential standards of acceptable nursing practice.  She resides in Redmond, Ore.
Douglas County
  • The Board revoked the practical nurse license of Amanda M. Catania (201230510LPN), of Sutherlin, Ore., for attempting to obtain unauthorized prescription drugs.
Grant County
  • The Board accepted the Voluntary Surrender of Laura A. Grant’s Registered Nurse license (200142161RN) for failing to comply with the terms and conditions of a Board Order.  She resides in Mount Vernon, Ore.
Jackson County
  • The Board suspended Wendy M. Lazaro’s nursing assistant certificate (201112074CNA) for a minimum of 14-days for failing to cooperate with the Board during an investigation.  She resides in Medford, Ore.
Klamath County
  • The Board accepted the Voluntary Surrender of Cynthia K. Moran’s Registered Nurse license (200540023RN) for failing to comply with the terms and conditions of a Board Order.  She resides in Bonanza, Ore.
Lincoln County
  • The Board accepted the Voluntary Surrender of Elaine B. Harley’s Registered Nurse license (081001286RN) for failing to take action to preserve client safety, improperly delegating tasks of nursing care to unlicensed persons, and failing to supervise persons to whom nursing tasks have been delegated.  She resides in Yachats, Ore.
Marion County
  • The Board revoked the nursing assistant and medication aide certificates of Hanna M. Brockamp (200910615CNA/201120036CMA), of Silverton, Ore., due to a Theft conviction and for removing money from the workplace without authorization.
  • The Board reprimanded Ross W. Cordes’s Registered Nurse license (098006707RN) with conditions for falsifying an agency record.  He resides in Salem, Ore.
Multnomah County
  • The Board accepted the Voluntary Surrender of John M. DeSimini’s Registered Nurse license (081012569RN) for using intoxicants to the extent injurious to himself or others.  He resides in Portland, Ore.
  • The Board revoked the nursing assistant and medication aide certificates of Mark L. Mitchell (201110214CNA/ 201320069CMA), of Gresham, Ore., for inaccurate and incomplete recordkeeping, falsifying medication administration records, failing to competently perform CNA duties, and failing to cooperate with the Board during an investigation.
  • The Board placed James Njuguna’s nursing assistant certificate (200712398CNA) on probation for 24-months for using intoxicants to the extent injurious to himself or others.  He resides in Portland, Ore.
Umatilla County
  • The Board suspended Shaunna L. Payne’s nursing assistant certificate (201112280CNA) for a minimum of 14-days for failing to cooperate with the Board during an investigation.  She resides in Pendleton, Ore.
Washington County
  • The Board placed Tamara L. Pfeifer’s Registered Nurse license (079043653RN) on probation for 24-months for practicing nursing while physically impaired, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Beaverton, Ore.
Out of State
  • Washington—The Board accepted the Voluntary Surrender of Shawna E. Hebert’s Registered Nurse license (200640222RN) for failing to comply with the terms and conditions of a Board Order.  She resides in Vancouver, Wash.
