Board of Nursing Disciplinary Actions Taken in August 2017
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 19 nurses or nursing assistants during its August 9, 2017, Board Meeting.

Clackamas County
  • The Board accepted the Voluntary Surrender of Cain D. Gurule’s practical nurse license (201230499LPN) for misrepresentation during the licensure process, inaccurate and incomplete recordkeeping, and failing to cooperate with the Board during the course of an investigation.  He resides in Sandy, Ore.
Coos County
  • The Board imposed a $700 civil penalty against Crystal D. Whited's registered nurse license (201243520RN) for practicing nursing without a current license.  She resides in Coos Bay, Ore.
Deschutes County
  • The Board accepted the Voluntary Surrender of Melissa A. Cranor’s registered nurse license (079037755RN) for practicing nursing when unable due to physical impairment.  She resides in Bend, Ore.
Lane County
  • The Board placed the registered nurse license of Alexandra V. Asencio (201404110RN) on probation for 24 months for violating the terms and conditions of the Health Professionals’ Services Program.  She resides in Springfield, Ore.
  • The Board revoked the registered nurse license of Lori A. McKay (096006037RN), of Lowell, Ore., for failing to comply with the terms and conditions of the Health Professionals’ Services Program, and using intoxicants to an extent or in a manner injurious to herself or others.
  • The Board revoked the nursing assistant certificate of Mandy L. Peck (201508630CNA), of Eugene, Ore., for abusing a person, engaging in sexual misconduct with a client, and failing to cooperate with the Board during the course of an investigation.
Marion County
  • The Board accepted the Voluntary Surrender of Douglas R. Miller’s registered nurse license (082011915RN) for implementing standards of care that jeopardize patient safety, and failing to conform to the essential standards of acceptable nursing practice.  He resides in Salem, Ore.
Multnomah County
  • The Board placed the practical nurse license of Lucy Bentea (201030416LPN) on probation for 24 months for using intoxicants to the extent or in a manner injurious to herself or others.  She resides in Portland, Ore.
Tillamook County
  • The Board revoked the nursing assistant certificate of Kim S. Siebern (200611303CNA), of Tillamook, Ore., for using intoxicants to the extent or in a manner injurious to herself or others, and failing to answer questions truthfully.
Umatilla County
  • The Board accepted the Voluntary Surrender of Mark E. Lanier’s nursing assistant certificate (201503321CNA) for abusing a person.  He resides in Pendleton, Ore.
Union County
  • The Board revoked the registered nurse license of Mary L. Blanc (200743470RN), of La Grande, Ore., for violating the terms and conditions of a Board Order.
  • The Board suspended the nursing assistant certificate of Anna L. Richards (200611680CNA) for 14 days for demonstrated incidents of violent, abusive, neglectful, or reckless behavior.  She resides in Hermiston, Ore.
Wallowa County
  • The Board reprimanded the registered nurse license of Glenda A. Cummins (200542347RN), of Enterprise, Ore., for failing to take action to preserve client safety based on nursing assessment and judgement, and failing to conform to the essential standards of acceptable nursing practice.
Washington County
  • The Board placed the nursing assistant certificate of Stacey M. Condray (201706572CNA) on probation for 24 months for using intoxicants to the extent or in a manner injurious to herself or others.  She resides in Tigard, Ore.
  • The Board revoked the registered nurse license of Tamara L. Pfeifer (079043653RN), of Beaverton, Ore., for failing to comply with the terms and conditions of the Health Professionals’ Services Program.
  • The Board revoked the registered nurse license of Rebecca C. Pulito (201141099RN), of Beaverton, Ore., for failing to comply with the terms and conditions of the Health Professionals’ Services Program.
Yamhill County
  • The Board placed the registered nurse license of Heather J. Russo (200440793RN) on probation for 24 months for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in McMinnville, Ore.
Out of State
  • Illinois—The Board accepted the voluntary withdrawal of Michael F. Cottrell’s RN application due to disciplinary actions taken in Arizona and Illinois.  He resides in Saint Charles, Ill.
  • New York—The Board suspended the registered nurse license of Elizabeth J. Kelly (201042459RN) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation.  She resides in Castleton on Hudson, N.Y.
