Board of Nursing Disciplinary Actions Taken in February 2013

Clackamas County
  • The Board accepted the voluntary surrender of Shannon L. Bradley’s Registered Nurse license (094003187RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Lake Oswego, Ore.
  • The Board placed Kimberly T. D. Joers’s Registered Nurse license (200641699RN) on probation for 24-months for inaccurate recordkeeping, Tampering with Patient Records conviction, and violating the terms and conditions of a Board Order.  She resides in Portland, Ore.
Clatsop County
  • The Board suspended Erik D. Jensen’s Registered Nurse license (093000371RN) for one-year, to be followed by 24-months of probation, for violating the terms and conditions of a Board Order.  He resides in Astoria, Ore.
Deschutes County
  • The Board accepted the voluntary surrender of Patricia A. Kolkow’s Registered Nurse license (081012597RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Bend, Ore.
  • The Board accepted the voluntary surrender of Amy D. Rondot’s practical nurse license (200630441LPN) for a Tampering with Drug Records conviction, failing to answer questions truthfully, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Redmond, Ore.
  • The Board placed Maureen M. Skatvold’s Registered Nurse license (097006043RN) on probation for 24-months for violating the terms and conditions of the Health Professionals’ Services Program.  She resides in Bend, Ore.
Douglas County
  • The Board accepted the voluntary surrender of Allen W. King’s Registered Nurse license (201041908RN) for falsifying a client record.  He resides in Roseburg, Ore.
  • The Board reprimanded Amber N. Potts’s Registered Nurse license (200941725RN) for failing to respect client dignity and rights, and failure to conform to the essential standards of acceptable nursing practice.  She resides in Winston, Ore.
  • The Board reprimanded Jessica Stevens’s nursing assistant certificate (201010274CNA) for client neglect.  She resides in Winston, Ore.
Jackson County
  • The Board reprimanded Barbara A. Rund’s Registered Nurse license (098003223RN) for failing to follow the plan of care.  She resides in Medford, Ore.
Jefferson County
  • The Board denied the CNA application of Michelle L. Allen (000038331CNA), of Madras, Ore., for using intoxicants to the extent injurious to herself or others.
Klamath County
  • The Board denied the CNA application of Jeanie A. Baker, of Chiloquin, Ore., for failing certification requirements.
Lane County
  • The Board suspended Joshua J. Sikes’s Registered Nurse license (200841341RN) for one-year, to be followed by 24-months of probation, for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  He resides in Junction City, Ore.
  • The Board revoked the Registered Nurse license of Lisa A. Cone (200541915RN), of Springfield, Ore., for failing to cooperate with the Board during an investigation.
  • The Board revoked the nursing assistant certificate of Martina M. Chisholm (200710456CNA), of Springfield, Ore., for an Identify Theft conviction and failing to cooperate with the Board during an investigation.
Lincoln County
  • The Board accepted the voluntary surrender of Melissa R. Metcalf’s Registered Nurse license (200342015RN) for violating the terms and conditions of a Board Order.  She resides in Newport, Ore.
  • The Board reprimanded Kelly M. Morris’s Registered Nurse license (201143355RN) with conditions for improperly delegating tasks of nursing care to unlicensed persons.  She resides in Lincoln City, Ore.
Linn County
  • The Board placed Renee M. Headington’s nursing assistant certificate (201310396CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Albany, Ore.
Marion County
  • The Board imposed a civil penalty in the amount of $3,750.00 on Jasen P. Henderson, of Salem, Ore., for being a nurse imposter.
  • The Board revoked the Registered Nurse license of Weldie Jean Simon (201240797RN), of Ocala, Ore., for fraud during the licensure process.
  • The Board reprimanded Elaine M. Harlan’s Registered Nurse license and Family Nurse Practitioner certificate (078040082RN/078040082N1) with conditions for failing to take action to preserve client safety.  She resides in Salem, Ore.
Multnomah County
  • The Board suspended Regenia D. Wilson’s nursing assistant certificate (200310886CNA) for 30-days with conditions for abuse and neglect of a client, and failing to respect client rights.  She resides in Portland, Ore.
  • The Board revoked the nursing assistant certificate of Momodu A. Turay (200410106CNA), of Portland, Ore., for client abuse, failing to respect client dignity, and failing to cooperate with the Board during an investigation.
  • The Board reprimanded Lori B. McMurtrey’s Registered Nurse license (094003136RN) for failing to teach and supervise unlicensed persons to whom nursing tasks had been delegated, incomplete recordkeeping, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Portland, Ore.
  • The Board reprimanded Monica R. Lohman’s nursing assistant certificate (201110426CNA) for client abuse and failing to perform CNA duties competently.  She resides in Portland, Ore.
  • The Board placed Etta M. Assuman’s nursing assistant certificate (201310401CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others, and failing to answer questions truthfully.  She resides in Gresham, Ore.
  • The Board placed Denise N. Marker’s nursing assistant certificate (201211725CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Fairview, Ore.
Polk County
  • The Board revoked the practical nurse license of Bailey A. Lawrence (201030109LPN), of Dallas, Ore., for using intoxicants to an extent injurious to herself or others, and failing to cooperate with the Board during an investigation.
  • The Board reprimanded Claudia E. Magruder’s nursing assistant certificate (200510563CNA) for misrepresentation during the certification process and failing to answer questions truthfully.  She resides in Salem, Ore.
Umatilla County
  • The Board revoked the practical nurse license of Amy B. Bray (L000912LPN), of Hermiston, Ore., for failing to comply with the terms and conditions of the Health Professionals’ Services Program.
  • The Board reprimanded Teresa I. McNeeley’s nursing assistant certificate (000021599CNA) for client neglect.  She resides in Milton Freewater, Ore.
  • The Board reprimanded Megan A. Simmons’s Registered Nurse license (200441210RN) for failing to conform to the essential standards of acceptable nursing practice.  She resides in Pendleton, Ore.
Washington County
  • The Board suspended Laura E. Wilson’s Registered Nurse license (087000408RN) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  She resides in Portland, Ore.
  • The Board placed Carey A. Christian’s Registered Nurse license (200841429RN) on probation for 24-months for Identity Theft and Forgery convictions, and obtaining unauthorized drugs.  She resides in Tigard, Ore.
Yamhill County
  • The Board suspended Ellen L. Brewster’s nursing assistant certificate (200412703CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  She resides in Sheridan, Ore.
Out of State
  • New York—The Board denied the RN application of Fouchard Ambroise, of Brooklyn, N.Y., for fraud during the licensure process and failing to answer questions truthfully.
  • New York—The Board denied the RN application of Angela L. Hickling, of Brooklyn, N.Y., for fraud during the licensure process and failing to answer questions truthfully.
  • New York—The Board denied the RN application of Natisha V. Morrison, of Brooklyn, N.Y., for fraud during the licensure process and failing to answer questions truthfully.
  • Washington—The Board placed Catherine G. Tahirovic’s Registered Nurse license (200742550RN) on probation for 24-months for unauthorized removal of drugs from the workplace, and using intoxicants to the extent injurious to herself or others.  She resides in Vancouver, Wash.
  • Washington—The Board reprimanded Melanie G. Casebolt’s Registered Nurse license (081001797RN) for inaccurate recordkeeping, and failure to conform to the essential standards of nursing practice.  She resides in Camas, Wash.
