Board of Nursing Disciplinary Actions Taken in February 2014

Clackamas County
  • The Board revoked the Registered Nurse license of Angela L. Perry (200241838RN), of Boring, Ore., for failing to comply with the terms and conditions of a Board Order, failing to answer questions truthfully, and failing to provide requested documents.
  • The Board reprimanded Josephe M. Vincent’s Registered Nurse license (200641180RN) for altering agency records and an Identity Theft conviction.  He resides in Canby, Ore.
  • The Board placed Vallyna D. Michaelis’s Registered Nurse license (098000107RN) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Happy Valley, Ore.
  • The Board revoked the Registered Nurse license of Christa R. Hallowell (200341535RN), of Boring, Ore., for failing to comply with the terms and conditions of the Health Professionals’ Services Program, and failing to cooperate with the Board during an investigation.
  • The Board accepted the voluntary surrender of Patrick J. Hallinan’s nursing assistant certificate (200811056CNA) for using intoxicants to the extent injurious to himself or others, and violating the terms and conditions of a Board Order.  He resides in Portland, Ore.
Clatsop County
  • The Board reprimanded Stacey I. Scruggs’s Registered Nurse license (077039062RN) for violating the terms and conditions of a Board Order.  She resides in Warrenton, Ore.
Douglas County
  • The Board reprimanded Mary J. McCurry’s Registered Nurse license (200141559RN) with conditions for inaccurate and incomplete recordkeeping, failing to take action to preserve client safety, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Riddle, Ore.
  • The Board accepted the voluntary surrender of Jane A. Huggins-Willer’s nursing assistant certificate (200010708CNA) for client abuse.  She resides in Reedsport, Ore.
Grant County
  • The Board placed Laura A. Grant’s Registered Nurse license (200142161RN) on probation for 24-months for the unauthorized removal of drugs from the workplace, and using intoxicants to the extent injurious to herself or others.  She resides in Mount Vernon, Ore.
Jackson County
  • The Board placed Debra L. Blackwell’s practical nurse license (077010047LPN) on probation for 24-months for practicing while impaired, and using intoxicants to the extent injurious to herself or others.  She resides in Medford, Ore.
  • Josephine County
  • The Board suspended Donald W. Richie’s Registered Nurse license (201143073RN) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  He resides in Cave Junction, Ore.
Lane County
  • The Board accepted the voluntary surrender of Lynda A. Weaver’s nursing assistant and medication aide certificates (200810177CNA/200920065CMA) for using intoxicants to the extent injurious to herself or others, and a physical condition that prevents her from performing CNA duties safely.  She resides in Eugene, Ore.
Linn County
  • The Board reprimanded Mary P. Shew’s nursing assistant certificate (000020168CNA) for failing to communicate information regarding client status, and failing to competently perform CNA duties.  She resides in Lebanon, Ore.
Marion County
  • The Board placed Sharon R. Rice’s Registered Nurse license (090000386RN) on probation for 12-months for inaccurate and incomplete recordkeeping, practicing nursing while impaired due to a physical condition, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Keizer, Ore.
  • The Board placed Julia A. Doherty’s Registered Nurse license (201340386RN) on probation for 24-months for using intoxicants to the extent injurious to himself or others.  She resides in Salem, Ore.
Multnomah County
  • The Board revoked the nursing assistant certificate of Esther R. Rosenshein (201010868CNA), of Portland, Ore., for leaving a CNA assignment without notifying appropriate supervisor, client neglect, falsifying an agency record, and failing to cooperate with the Board during an investigation.
  • The Board revoked the Registered Nurse license of Mark D. Elliott (200041437RN), of Portland, Ore., for using intoxicants to the extent injurious to himself or others, failing to conform to the essential standards of acceptable nursing practice, and failing to cooperate with the Board during an investigation.
Polk County
  • The Board suspended Patrick D. Smith’s nursing assistant certificate (200910588CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  He resides in Rickreall, Ore.
Umatilla County
  • The Board accepted the voluntary surrender of Jerrod T. Mikel’s nursing assistant certificate (000008870CNA) for violating the terms and conditions of a Board Order.  He resides in Pilot Rock, Ore.
Washington County
  • The Board revoked the practical nurse license of Brian S. Renhard (201230022LPN), of Tualatin, Ore., for using intoxicants to the extent injurious to himself and others, and failing to cooperate with the Board during an investigation.
  • The Board placed Kathy M. Florian’s nursing assistant certificate (201400980CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Forest Grove, Ore.
Out of State
  • California—The Board denied Inocencia C. DeVargas’s practical nurse application for failing to meet educational requirements for licensure.  She resides in San Jose, Calif.
  • Illinois—The Board denied Regina K. Syjuco’s practical nurse application for failing to meet educational requirements for licensure.  She resides in Skokie, Ill.
  • Nevada—The Board denied Stanley T. Cachapero’s practical nurse application for failing to meet educational requirements for licensure.  He resides in Reno, Nev.
