Board of Nursing Disciplinary Actions Taken in February 2017
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 28 nurses or nursing assistants during its February 15, 2017, Board Meeting.

Coos Bay County
  • The Board reprimanded the registered nurse license of Ronda L. Taylor (099003020RN), of North Bend, Ore., for the unauthorized removal of supplies from the workplace, and implementing standards of care that jeopardize patient safety.
Douglas County
  • The Board accepted the Voluntary Withdrawal of Maren J. Jetmore’s CNA application (200212373CNA) for using intoxicants to an extent or in a manner injurious to herself or others, and misrepresentation during the certification process.  She resides in Roseburg, Ore.
  • The Board accepted the Voluntary Withdrawal of Douglas R. Lally’s RN application (083042170RN) for using intoxicants to an extent or in a manner injurious to himself or others, and failing to answer questions truthfully.  He resides in Camas Valley, Ore.
Jackson County
  • The Board accepted the Voluntary Surrender of Evan P. Mikkelson’s nursing assistant certificate (201609353CNA) for violating the terms and conditions of a Board Order.  He resides in Talent, Ore.
  • The Board suspended the nursing assistant certificate of Laura K. Secor (201112332CNA) for 30 days for violating a person’s rights to privacy and confidentiality.  She resides in Medford, Ore.
Josephine County
  • The Board denied the RN application of Danitra R. Brown (200942023RN), of Grants Pass, Ore., for failing to answer questions truthfully and using intoxicants to an extent or in a manner injurious to herself or others.
  • The Board reprimanded the nursing assistant certificate of Adrien A. Romano (201500531CNA), of Grants Pass, Ore., for violating a person’s rights of privacy and confidentiality.
Lane County
  • The Board placed the registered nurse license of Danielle A. N. Halbrook (201242432RN) on probation for 36 months for using intoxicants to the extent injurious or in a manner dangerous to herself or others.  She resides in Eugene, Ore.
  • The Board reprimanded the registered nurse license of Trena M. Lindsey (098000397RN), of Eugene, Ore., for incomplete recordkeeping and failing to maintain timely records.
  • The Board denied the CNA application of Hermelinda Sprague (200710468CNA), of Eugene, Ore., for using intoxicants to the extent or in a manner injurious to herself or others, and failing to cooperate during the course of an investigation.
Marion County
  • The Board suspended the nursing assistant certificate of Estela Najar (200611733CNA) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation. She resides in Salem, Ore.
  • The Board reprimanded the registered nurse license of Nora R. Willow (201041923RN), of Woodburn, Ore., for failing to communicate timely information regarding client status to members of the healthcare team, and failing to modify the plan of care as needed.
Multnomah County
  • The Board accepted the Voluntary Surrender of Stephan R. Bayley’s registered nurse license (200642222RN) for gross incompetence, unauthorized removal of client information from the workplace, performing acts beyond his authorized scope, and failing to conform to the essential standards of acceptable nursing practice.  He resides in Portland, Ore.
  • The Board placed the registered nurse license of Danielle A. Bishop (201341775RN) on probation for 24 months for using intoxicants to the extent injurious or in a manner dangerous to herself or others.  She resides in Portland, Ore.
  • The Board accepted the Voluntary Surrender of Sue A. Plass’s registered nurse license (083037937RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Gresham, Ore.
  • The Board placed the registered nurse license of Frank D. Rickman (200340700RN) on probation for 24 months for using intoxicants to the extent injurious or in a manner dangerous to himself or others.  He resides in Portland, Ore.
  • The Board denied the RN application of Karla J. Surfus (086000370RN), of Portland, Ore., for using intoxicants to the extent or in a manner injurious to herself or others.
Polk County
  • The Board accepted the Voluntary Surrender of Laurel A. Allen’s registered nurse license (200941689RN) for a physical condition that makes her unable to practice nursing safely and violating the terms and conditions of a Board Order.  She resides in Salem, Ore.
  • The Board accepted the Voluntary Surrender of Christine L. Davis’s registered nurse license (098007334RN) for violating the terms and conditions of a Board Order.  She resides in Dallas, Ore.
  • The Board reprimanded the nursing assistant certificate of Angela R. Hussey (200810049CNA), of Salem, Ore., for jeopardizing the safety of a person under her care.
  • The Board suspended the registered nurse license and family nurse practitioner certificate of Joancarole J. Sochin (087006857RN/087006857N1) for 30 days for failing to respect the dignity and rights of clients, regardless of national origin or nature of health needs.  She resides in Dallas, Ore.
Umatilla County
  • The Board placed the nursing assistant certificate of Wendy G. Rodriguez (201604784CNA) on probation for 12 months for conviction of a crime that bears demonstrable relationship to CNA duties.  She resides in Hermiston, Ore.
Washington County
  • The Board revoked the registered nurse license of Joan M. Cairns (088000362RN), of Hillsboro, Ore., for using intoxicants to the extent or in a manner injurious to herself or others.
  • The Board accepted the Voluntary Withdrawal of Laura R. Finney’s RN application (082011822RN) due to impairment.  She resides in Beaverton, Ore.
Yamhill County
  • The Board imposed a $5,000 civil penalty against Cheyenne R. Marrs’s certified nursing assistant certificate (000041709CNA) for being a nurse imposter.  She resides in Dundee, Ore.
Out of State
  • California—The Board accepted the Voluntary Surrender of Dina M. Dean’s registered nurse license (201141780RN) for violating the terms and conditions of a Board Order.  She resides in San Diego, Calif.
  • Idaho—The Board placed an emergency suspension on Kirt S. Adams’s registered nurse license (201141044RN) for incomplete recordkeeping, practicing nursing while unable due to psychological or mental impairment, failing to conform to the essential standards of acceptable nursing practice, and failing to cooperate with the Board during an investigation.  He resides in Fruitland, Idaho.
  • Washington—The Board revoked the registered nurse license of Markeatha Suddreth (200040787RN), of Vancouver, Wash., for violating the terms and conditions of a Board Order.
