Board of Nursing Disciplinary Actions Taken in January 2013
 
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 43 nurses or nursing assistants during its January 16, 2013, Board Meeting.

Clackamas County
  • The Board accepted the voluntary surrender of Trena S. Sutton’s practical nurse license (200030227LPN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Milwaukie, Ore.
  • The Board revoked the Registered Nurse license of Kathrin J. Schoppert (200341158RN), of Rhododendron, Ore., for failing to comply with the terms and conditions of the Health Professionals’ Services Program.
  • The Board reprimanded Regina A. Curtis’ Registered Nurse license (084060988RN) with conditions for failing to supervise persons to whom nursing tasks have been assigned, failing to report actual or suspected incidents of abuse, failing to communicate information regarding client status, and failing to take action to preserve client safety.  She resides in Oregon City, Ore.
Curry County
  • The Board accepted the voluntary surrender of Jennifer L. Quesada’s Registered Nurse license (200742059RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Gold Beach, Ore.
  • The Board reprimanded Stacy R. Kreger’s Registered Nurse license (200540772RN) for client neglect and failing to take action to promote client safety.  She resides in Brookings, Ore.
Jackson County
  • The Board revoked the nursing assistant certificate of Jillian J. Hartman (201113133CNA), of Medford, Ore., for removing property from the workplace without authorization, and failing to cooperate with the Board during the course of an investigation.
  • The Board reprimanded Elaine M. Dunn’s Registered Nurse license (092003262RN) with conditions for failing to maintain client records in a timely manner, performing acts beyond her authorized scope, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Medford, Ore.
  • The Board reprimanded Annette C. Tobiason’s nursing assistant certificate (200812702CNA) for failing to perform CNA duties competently.  She resides in Medford, Ore.
  • The Board reprimanded Shannon Dell Rio’s Registered Nurse license and Family Nurse Practitioner certificate (085074775RN/085074775N1) for failing to report actual or suspected incidents of abuse to the appropriate state authorities, and failing to take action to preserve or promote client safety.  She resides in Ashland, Ore.
  • The Board reprimanded Cathy J. McGinnis’ nursing assistant certificate (000037169CNA) for client neglect.  She resides in Medford, Ore.
Klamath County
  • The Board revoked the Registered Nurse license of Cheryl L. Knutson (095000385RN), of Klamath Falls, Ore., for obtaining drugs without authorization, and using intoxicants to the extent injurious to herself or others.
Lane County
  • The Board placed Elbridge Gerry J. Louthan’s nursing assistant certificate (201112980CNA) on probation for 24-months for using intoxicants to the extent injurious to himself or others.  He resides in Eugene, Ore.
  • The Board accepted the voluntary surrender of Judith A. Fowler Langston’s Registered Nurse license (086000289RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Eugene, Ore.
  • The Board accepted the voluntary surrender of Thaddeus R. Kwiecinski’s Registered Nurse license (078041306RN) for possessing unauthorized prescription drugs.  He resides in Eugene, Ore.
  • The Board suspended Martina M. Chisholm’s nursing assistant certificate (200710456CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  She resides in Springfield, Ore.
  • The Board revoked the Registered Nurse license of Mirian A. Hourigan (200742249RN), of Eugene, Ore., for failing to comply with the terms and conditions of the Health Professionals’ Services Program.
  • The Board revoked the nursing assistant certificate of June D. Hooper (200211587CNA), of Eugene, Ore., for client abuse, and failing to respect client rights and dignity.
  • The Board reprimanded April M. Miotke’s practical nurse license (201130524LPN) for failing to develop, implement and/or follow through with the plan of care, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Cottage Grove, Ore.
  • The Board reprimanded Dana R. Jennings’ nursing assistant certificate (201011365CNA) for using the client relationship for personal gain.  She resides in Eugene, Ore.
Linn County
  • The Board suspended Christina S. E. Schwend’s nursing assistant certificate (200510719CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  She resides in Albany, Ore.
Marion County
  • The Board accepted the voluntary surrender of Jennifer L. Lucas’ Registered Nurse license (200641662RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Salem, Ore.
Multnomah County
  • The Board placed the license of RN applicant Rebecca A. Forte on probation for 24-months, pending successful completion of a reentry program, for using intoxicants to the extent injurious to herself or others.  She resides in Portland, Ore.
  • The Board suspended Momodu S. Turay’s nursing assistant certificate (200410106CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  He resides in Portland, Ore.
  • The Board reprimanded Shon P. Lochrie’s nursing assistant certificate (000034036CNA) and accepted the voluntary surrender of her medication aide certificate (000034036CMA) for incomplete and inaccurate recordkeeping, and performing acts beyond her authorized duties.  She resides in Portland, Ore.
  • The Board placed Jennifer A. Davis’ nursing assistant certificate (000038800CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Troutdale, Ore.
Polk County
  • The Board revoked the nursing assistant certificate of Sergio H. Gonzalez (201113288CNA), of Falls City, Ore., for client abuse, and failing to perform CNA duties competently.
Umatilla County
  • The Board accepted the voluntary surrender of Cher L. Nycz’s nursing assistant certificate (200812616CNA) for client neglect, and failing to respect client rights and dignity.  She resides in Hermiston, Ore.
  • The Board suspended Candace J. Sturtevant’s practical nurse license (087003224LPN) for a minimum of 14 days, for failing to maintain tax compliance with the Oregon Department of Revenue (per ORS 305.385).  She resides in Helix Freewater, Ore.
  • The Board suspended Laura A. Koskenmaki’s Registered Nurse license (096007347RN) for a minimum of 14 days, for failing to maintain tax compliance with the Oregon Department of Revenue (per ORS 305.385).  She resides in Milton Freewater, Ore.
  • The Board revoked the nursing assistant certificate of Rogeanna S. Hug (000028951CNA), of Milton Freewater, Ore., for client neglect, failing to communicate information regarding the client status, and failing to perform CNA duties competently.
Washington County
  • The Board accepted the voluntary surrender of Rachel A. Harp’s nursing assistant certificate (200542222RN) for multiple convictions and possessing unauthorized drugs.  She resides in Beaverton, Ore.
  • The Board accepted the voluntary surrender of Holly H. Rudick’s nursing assistant certificate (000040074CNA) for exploiting the client relationship for personal gain, and failing to respect client rights and dignity.  She resides in Beaverton, Ore.
  • The Board revoked the Registered Nurse license of Rita L. Sedivy (077038879RN), of Beaverton, Ore., for using intoxicants to the extent injurious to herself or others, and failing to provide requested documents.
  • The Board revoked the nursing assistant and medication aide certificates of Andrea M. Abshire (200711137CNA/200820087CMA), of Beaverton, Ore., for using intoxicants to the extent injurious to herself or others.
  • The Board reprimanded Monica T. Stutzman’s Registered Nurse license (200441296RN) for incomplete and inaccurate recordkeeping, and failing to follow through with the plan of care.  She resides in Hillsboro, Ore.
  • The Board placed Jennifer Aamodt’s nursing assistant certificate (201010333CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Hillsboro, Ore.
Yamhill County
  • The Board accepted the voluntary surrender of Ariel C. Reed’s nursing assistant certificate (200612576CNA) for using intoxicants to the extent injurious to herself or others, and failing to cooperate with the Board during an investigation.  She resides in McMinnville, Ore.
  • The Board denied the RN application of Vicki R. Keller (089003209RN), of McMinnville, Ore., for failing to answer questions truthfully, using intoxicants to the extent injurious to herself or others, failing to conform to the essential standards of acceptable nursing practice, and failing to cooperate with the Board during an investigation.
Out of State
  • Colorado—The Board denied the RN application of Peter G. Oakeshott, of Colorado Springs, Colo., for exceeding the three-year limit granted for testing following graduation or program completion.
  • Colorado—The Board accepted the voluntary surrender of Samantha K. Zomar’s Registered Nurse license (200940743RN) for violating the terms and conditions of a Board Order.  She resides in Denver, Colo.
  • Massachusetts—The Board accepted the voluntary surrender of Regina G. Downing’s Registered Nurse license (201142996RN) for unauthorized removal of narcotics from the workplace, and practicing nursing while impaired.  She resides in East Harwich, Mass.
  • Michigan—The Board revoked the nursing assistant certificate of Bonnie J. Sheller (201010049CNA), of Grand Rapids, Mich., for failing to provide requested documents and failing to cooperate with the Board during an investigation.
  • Washington—The Board placed Tamara S. Sharp’s Registered Nurse license (200341459RN) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Vancouver, Wash.
