Board of Nursing Disciplinary Actions Taken in January 2014

Clackamas County
  • The Board placed Timothy R. Hughes’ nursing assistant certificate (201210783CNA) on probation for 24-months for using intoxicants to the extent injurious to himself or others.  He resides in Portland, Ore.
Columbia County
  • The Board suspended Cheryl R. Wright’s nursing assistant certificate (200711912CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Saint Helens, Ore.
Douglas County
  • The Board denied Brandy L. Granziero’s practical nurse application (099005058LPN) due to fraud during the licensure process, failing to provide requested documents, and failing to answer questions truthfully.  She resides in Riddle, Ore.
Jackson County
  • The Board placed Kara B. Stuart’s Registered Nurse license (200941108RN) on probation for 24-months for the unauthorized removal of drugs from the workplace, client exploitation, failing to dispense or administer medications, incomplete and inaccurate recordkeeping, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Shady Cove, Ore.
  • The Board placed Helen J. Banuelo’s practical nurse license (083042738LPN) on probation for 24-months for using intoxicants to the extent injurious to herself or others, performing acts beyond her authorized scope, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Talent, Ore.
  • The Board revoked the Registered Nurse license of Carrie L. Christie (200742530RN), of Medford, Ore., for possessing controlled drugs, using intoxicants to the extent injurious to herself and others, and failing to cooperate with the Board during an investigation.
Josephine County
  • The Board denied Sarah H. Wilson’s LPN application due deceit in the practice of nursing, and practicing nursing without a license.  She resides in Grants Pass, Ore.
  • The Board suspended Sharon M. Maw’s Registered Nurse license (098006463RN) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Wilderville, Ore.
Lane County
  • The Board revoked the nursing assistant certificate of Karrissa J. Plaza (200910757CNA), of Eugene, Ore., for using intoxicants to the extent injurious to herself or others.
Linn County
  • The Board reprimanded Theresa J. Edmiston’s Registered Nurse license (099000832RN) for directing another person to falsify records, failing to answer questions truthfully, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Albany, Ore.
Marion County
  • The Board reprimanded Brandy L. Bell’s nursing assistant certificate (200412980CNA) for failing to perform CNA duties competently.  She resides in Salem, Ore.
Multnomah County
  • The Board accepted the voluntary surrender of Cindy L. Rumble’s Registered Nurse license (083042430RN) for the unauthorized removal of drugs from the workplace, and using intoxicants to the extent injurious to herself or others.  She resides in Portland, Ore.
  • The Board suspended Esther R. Rosenshein’s nursing assistant certificate (201010868CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Portland, Ore.
  • The Board suspended Adam V. Moore’s practical nurse license (200530007LPN) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  He resides in Portland, Ore.
  • The Board suspended Mark L. Mitchell’s nursing assistant and medication aide certificates (201110214CNA/201320069CMA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  He resides in Gresham, Ore.
  • The Board reprimanded Thomas E. Hill’s nursing assistant certificate (201310598CNA) for failing to respect client rights and dignity, engaging in sexual misconduct related to a client, and client neglect.  He resides in Portland, Ore.
  • The Board suspended Brooklyn T. L. Engelbert’s nursing assistant and medication aide certificates (200612102CNA/201020062CMA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Portland, Ore.
  • The Board suspended Mark D. Elliott’s Registered Nurse license (200041437RN) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  He resides in Portland, Ore.
Washington County
  • The Board placed Myka L. Jewell’s Registered Nurse license (200642284RN) on probation for 24-months for failure to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Beaverton, Ore.
  • The Board suspended Brian S. Renhard’s practical nurse license (201230022LPN) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  He resides in Tualatin, Ore.
  • The Board suspended Cheelo N. Kangar’s nursing assistant certificate (201010436CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Portland, Ore.
Yamhill County
  • The Board revoked the nursing assistant and medication aide certificates of Sandy L. Waggoner (000031476CNA/200120159CMA), of Newberg, Ore., for failing to administer medications as ordered, failing to document medications as administered, possessing unauthorized drugs, and failing to cooperate with the Board during an investigation.
  • The Board suspended Jennifer N. Kadell’s Registered Nurse license (200942108RN) for 60 days with conditions for failing to maintain professional boundaries with a client, engaging in other unacceptable behavior toward a client, violating client privacy, and failing to answer questions truthfully.  She resides in McMinnville, Ore.
Out of State
  • Arizona—The Board suspended Debra R. Leslie-Augustus’ Psychiatric Mental Health Nurse Practitioner certificate (200650079NP) for 60 days for deceit in the practice of nursing, implementing standards of care which jeopardized patient safety, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Sedona, Ariz.
  • Georgia—The Board placed Chad S. Ellsworth’s Registered Nurse license (201400317RN) on probation for 24-months for using intoxicants to the extent injurious to himself or others, and practicing nursing while impaired.  He resides in Atlanta, Ga.
  • North Dakota—The Board reprimanded James W. Green’s Registered Nurse license (201243535RN) for inaccurate and incomplete recordkeeping, failing to maintain client records in a timely manner, and failing to conform to the essential standards of acceptable nursing practice.  He resides in Fargo, N.D.
  • Washington—The Board revoked the Registered Nurse license of Cathy A. Scalzo (098007065RN), of Naselle, Wash., for practicing when unable to perform due to a mental impairment.
  • Washington—The Board accepted the voluntary surrender of Dena R. Conklin’s Registered Nurse license (201243500RN) obtaining unauthorized prescription drugs.  She resides in Kennewick, Wash.
  • Washington—The Board revoked the Registered Nurse license of Dawn M. Frazier (096006696RN), of Vancouver, Wash., for possessing unauthorized drugs, failing to conform to the essential standards of acceptable nursing practice, and failing to cooperate with the Board during an investigation.
  • Washington—The Board denied Ashley M. Mohon’s nursing assistant application for using intoxicants to the extent injurious to herself or others.  She resides in Ocean Park, Wash.
