Board of Nursing Disciplinary Actions Taken in January 2017
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 26 nurses or nursing assistants during its January 18, 2017, Board Meeting.

Clackamas County
  • The Board suspended the nursing assistant certificate of Jeffrey S. Brumbaugh (000023976CNA) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation. He resides in Molalla, Ore.
  • The Board revoked the registered nurse license and family nurse practitioner certificate of Julie A. Demille (201242936RN/201250158NP), of Estacada, Ore., for inaccurate and incomplete recordkeeping, prescribing drugs for other than therapeutic or prophylactic purposes, failing to properly assess and document client assessment, charging a client in a grossly negligent manner, and failing to answer questions truthfully.
Coos County
  • The Board accepted the Voluntary Surrender of Susanne Ellenberger’s practical nurse license (200630014LPN) for violating the terms and conditions of a Board Order. She resides in Coquille, Ore.
  • The Board reprimanded the nursing assistant certificate of Sara M. Garcia (201012461CNA), of Coos Bay, Ore., with conditions for the unauthorized removal of supplies from any person or setting.
Deschutes County
  • The Board accepted the withdrawal of Kimberly S. Adkins’s CNA application for conviction of a crime that bears demonstrable relationship to duties of a CNA. She resides in Bend, Ore.
  • The Board suspended the registered nurse license of Kimberly D. Webster (200542054RN) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation. She resides in Redmond, Ore.
Jackson County
  • The Board suspended the nursing assistant certificate of Cindy A. Anderson (000015944CNA) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation. She resides in Central Point, Ore.
  • The Board revoked the nursing assistant and medication aide certificates of Mollie A. Schweinfurter (000043064CNA/200120124CMA), of White City, Ore., for performing CNA duties while impaired and failing to cooperate with the Board during the course of an investigation.
Josephine County
  • The Board suspended the nursing assistant certificate of Jody R. Fulmer (201401744CNA) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation. She resides in Grants Pass, Ore.
  • The Board reprimanded the family nurse practitioner certificate of Raymond A. Millette (095000636N1), of Grants Pass, Ore., for prescribing drugs in an unsafe manner, implementing standards of nursing practice that jeopardize patient safety, and failing to conform to the essential standards of acceptable nursing practice.
Lane County
  • The Board accepted the Voluntary Surrender of Margaret E. Hicks’s registered nurse license (091003077RN) for possessing unauthorized drugs, and using intoxicants to the extent injurious to herself or others. She resides in Springfield, Ore.
  • The Board suspended the nursing assistant certificate of Ruth E. Millard (201011441CNA) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation. She resides in Springfield, Ore.
  • The Board accepted the Voluntary Surrender of Peter H. Speerstra’s registered nurse license (200742068RN) for violating the terms and conditions of a Board Order. He resides in Florence, Ore.
Linn County
  • The Board reprimanded the registered nurse license of Neil Ruthven (201406397RN), of Lebanon, Ore., for client abuse and failing to respect client dignity and rights.
Marion County
  • The Board accepted the Voluntary Surrender of Jason W. Hofmann’s registered nurse license (201040888RN) for inaccurate and incomplete recordkeeping, unauthorized removal of drugs from the workplace, and a conviction for Tampering with Drug Records. He resides in Stayton, Ore.
Multnomah County
  • The Board revoked the nursing assistant certificate of Jennifer L. Ganoe (200412875CNA), of Portland, Ore., due to a conviction for Attempted Unlawful Delivery of a Controlled Substance.
  • The Board revoked the registered nurse license of Janet Porter (095000597RN), of Gresham, Ore., for failing to report incidents of child abuse to the appropriate state authorities and a Criminal Mistreatment conviction.
  • The Board accepted the withdrawal of Michele L. Renninger’s RN application for using intoxicants to the extent or in a manner injurious to herself or others. She resides in Portland, Ore.
Tillamook County
  • The Board revoked the registered nurse license of Daniel R. Sparks (099006452RN), of Beaver, Ore., for conviction of a crime that bears demonstrable relationship to the practice of nursing.

Union County

  • The Board placed the registered nurse license of Mary L. Blanc (200743470RN) on probation for 24 months for using intoxicants to the extent injurious or in a manner dangerous to herself or others. She resides in La Grande, Ore.
Washington County
  • The Board placed the registered nurse license of Ladybyrd Wong (201341496RN) on probation for 12 months for inaccurate recordkeeping, failing to respect client dignity and rights, and implementing standards of care that jeopardize patient safety. She resides in Beaverton, Ore.
Out of State
  • Louisiana— The Board revoked the registered nurse license of Velvet Jeter (201602778RN), of Woodworth, La., for practicing in Oregon without a valid license and unauthorized removal of property from the workplace.
  • New Mexico—The Board revoked the registered nurse license of Kendra L. Furgason (201506334RN), of Santa Fe, N.M., for inaccurate and incomplete recordkeeping, failing to take action to promote client safety, practicing nursing while impaired, and failing to conform to the essential standards of acceptable nursing practice.
  • Tennessee— The Board accepted the Voluntary Surrender of Jessica L. Hardy’s registered nurse license (201506903RN) for obtaining unauthorized prescription drugs. She resides in Memphis, Tenn.
  • Washington— The Board reprimanded the registered nurse license of Courtney K. Chipman (200842380RN), of Washougal, Wash., for willful misrepresentation in applying for a license and failing to answer questions truthfully.
  • Washington—The Board placed the registered nurse license of Lynn M. Counts (096007077RN) on probation for 12 months for inaccurate and incomplete recordkeeping, and failing to follow through with the plan of care. She resides in Goldendale, Wash.
