Board of Nursing Disciplinary Actions Taken in July 2012

Clackamas County
  • The Board accepted the voluntary surrender of Jennifer L. Klaus’ Registered Nurse license (200641562RN) for violating the terms and conditions of a Board Order.  She resides in Gladstone, Ore.
  • The Board accepted the voluntary surrender of Neil Larocco’s Registered Nurse license (096006633RN) for violating the terms and conditions of a Board Order.  He resides in Clackamas, Ore.
  • The Board revoked the Registered Nurse license of Susan E. Kessner (093000566RN), of West Linn, Ore., for failing to cooperate with the Board during an investigation.
  • The Board placed Aleksandr A. Voloshin’s nursing assistant certificate (200410930CNA) on probation for 24-months for using intoxicants to the extent injurious to himself or others.  He resides in Damascus, Ore.
Columbia County
  • The Board revoked the Registered Nurse license of Teresa M. Martinolich (097003211RN), of Clatskanie, Ore., for using intoxicants to the extent injurious to herself or others, and failing to answer questions truthfully.
Coos County
  • The Board accepted the voluntary surrender of Michelle M. Dieckman’s Registered Nurse license (200241529RN) for violating the terms and conditions of the Nurse Monitoring Program.  She resides in North Bend, Ore.
  • The Board suspended Brittany D. Morgan’s nursing assistant certificate (200410506CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  She resides in North Bend, Ore.
Deschutes County
  • The Board suspended Camille C. Pedersen’s nursing assistant certificate (201110384CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  She resides in Bend, Ore.
Jackson County
  • The Board accepted the voluntary surrender of Gayla F. Vick’s practical nurse license (075008220LPN) for violating the terms and conditions of a Board Order.  She resides in Medford, Ore.
  • The Board reprimanded the nursing assistant certificate of Hannah L. Fyre (200910251CNA) using intoxicants to the extent injurious to herself or others.  She resides in Jacksonville, Ore.
Klamath County
  • The Board placed the CNA certificate of applicant Jeanie A. Baker on probation for 24-months for using intoxicants to the extent injurious to himself or others.  She resides in Chiloquin, Ore.
Lane County
  • The Board revoked the nursing assistant certificate of Shari L. Clark (200513005CNA), of Eugene, Ore., for using intoxicants to the extent injurious to herself or others, and violating the terms and conditions of a Board Order.
  • The Board revoked the nursing assistant certificate of Shantelle R. Lawrence (200211417CNA), of Junction City, Ore., for using intoxicants to the extent injurious to herself or others.
  • The Board placed Sharry D. Janz’s Registered Nurse license (080012227RN) on probation for 24-months for the unauthorized removal of drugs from the workplace, and for using intoxicants to the extent injurious to herself or others.  She resides in Veneta, Ore.
Marion County
  • The Board revoked the nursing assistant certificate of Maria Del Carmen-Picasso (000037096CNA), of Silverton, Ore., for failing to respect client rights, client abuse and neglect, and failing to cooperate with the Board during an investigation.
Multnomah County
  • The Board suspended Shameaca Love Phillips’ nursing assistant certificate (200712729CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  She resides in Gresham, Ore.
  • The Board suspended Marlee A. Clendenon’s nursing assistant certificate (200910287CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  She resides in Portland, Ore.
Polk County
  • The Board accepted the voluntary withdrawal of Margaret Norton-Baihuber’s RN application for failing to meet educational requirements for licensure.  She resides in Independence, Ore.
Umatilla County
  • The Board suspended Angelina M. Kinsey’s nursing assistant certificate (200711619CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  She resides in Milton Freewater, Ore.
Wasco County
  • The Board accepted the voluntary surrender of Angela R. Lindell’s nursing assistant certificate (201010990CNA) due to Sexual Harassment and Contributing to the Sexual Delinquency of a Minor convictions.  She resides in Dufur, Ore.
Washington County
  • The Board reprimanded the practical nurse license of Noreen M. Gibbons (091005007LPN) for failing to conform to the essential standards of acceptable nursing practice.  She resides in Portland, Ore.
Out of State
  • Washington—The Board denied the RN application of Karen L. Jenkins, of Lake Forest Park, Wash., for using intoxicants to the extent injurious to herself or others, and failing to answer questions truthfully during the licensure process.
  • Washington—The Board reprimanded the practical nurse license of Tara L. Carlson (201230100LPN) for inaccurate recordkeeping, failing to communicate information regarding the client’s status, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Vancouver, Wash.
  • Washington—The Board suspended Shelley D. Tabe Arrey’s Registered Nurse license (200843023RN) for 180 days with conditions for performing acts beyond her authorized scope, failing to take action to preserve client safety, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Vancouver, Wash.
