Board of Nursing Disciplinary Actions Taken in July 2013
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 26 nurses or nursing assistants during its July 17, 2013, Board Meeting.

Clackamas County
  • The Board accepted the voluntary surrender of Jeff M. Marshall’s nursing assistant certificate (201012625CNA) for using intoxicants to the extent injurious to himself or others, and exploiting the client relationship for personal gain.  He resides in Happy Valley, Ore.
  • The Board revoked the nursing assistant certificate of Laura C. Firkus (000025483CNA), of Gladstone, Ore., for failing to comply with the terms and conditions of a Board Order.
  • The Board suspended Belkis K. Basulto’s nursing assistant certificate (201013021CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Clackamas, Ore.
Columbia County
  • The Board denied the Registered Nurse and Pediatric Nurse Practitioner applications of Connie M. Davis (000038358RN/000038358N2), of Westport, Ore., for performing acts beyond her authorized scope, prescribing medications to clients who were not within her scope of practice, and failing to answer questions truthfully.
Coos County
  • The Board suspended Wendi A. Schaefer’s Registered Nurse license (091007383RN) for three months with conditions, to be followed by 36 months of probation for inaccurate and incomplete recordkeeping, failing to maintain professional boundaries, failing to communicate information regarding client status, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Coos Bay, Ore.
  • The Board accepted the voluntary surrender of Tiffany R. Green’s Registered Nurse license (201043166RN) for using intoxicants to the extent injurious to herself or others and misrepresentation during the licensure process.  She resides in Bandon, Ore.
Deschutes County
  • The Board placed Patty Cavanagh’s limited Registered Nurse license on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Bend, Ore.
Douglas County
  • The Board suspended Susan L. Driver’s Registered Nurse license (093003352RN) for one year, to be followed by 24-months of probation for practicing nursing while impaired, inaccurate recordkeeping, unauthorized removal of drugs from the workplace, and using intoxicants to the extent injurious to herself or others.  She resides in Roseburg, Ore.
Jackson County
  • The Board suspended Penny S. Saul’s nursing assistant certificate (200810081CNA) for 30 days for failing to respect client dignity and client abuse.  She resides in Medford, Ore.
  • The Board placed Carrie C. Meier’s nursing assistant certificate (200510836CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Medford, Ore.
  • The Board denied Kellie C. Reilly’s CNA application for using intoxicants to the extent injurious to herself or others.  She resides in Jacksonville, Ore.
Lane County
  • The Board revoked the Registered Nurse license of Shelly D. Waggoner (083042652RN), of Springfield, Ore., for the unauthorized removal of property from any person.
  • The Board denied the RN application of Glenda J. Smail (092003284RN), of Springfield, Ore., for having been disciplined in another state, and supplying misleading information on an application.
  • The Board reprimanded Kimberly D. Robles’s Registered Nurse license (200440951RN) for Assault and Reckless Endangerment convictions.  She resides in Eugene, Ore.
Linn County
  • The Board revoked the nursing assistant certificate of Lacey L. Lawrence (201212144CNA), of Mill City, Ore., for client abuse, and failing to cooperate with the Board during an investigation.
  • The Board suspended Maegen J. Lawler’s nursing assistant certificate (201013180CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Lebanon, Ore.
Marion County
  • The Board suspended Louis J. Schippers’s Registered Nurse license (078041530RN) for six months for failing to take action to promote client safety, incomplete recordkeeping, failing to maintain client records in a timely manner, and failing to conform to the essential standards of acceptable nursing care.  He resides in Salem, Ore.
  • The Board suspended Alaina C. Jezza’s nursing assistant certificate (000030821CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Salem, Ore.

Multnomah County

  • The Board suspended Joshua Rodriguez-Ruiz’s practical nurse license (201130367LPN) for 30 days for an Assault of a Public Safety Officer conviction.  He resides in Portland, Ore.

Polk County

  • The Board suspended Deborah R. Yager’s nursing assistant certificate (000021755CNA) for 60 days with conditions for failing to respect client rights, client abuse, and failing to perform CNA duties competently.  She resides in Salem, Ore.
Umatilla County
  • The Board placed Jerrod T. Mikel’s nursing assistant certificate (000008870CNA) on probation for 24-months for misrepresentation during the certification process, and using intoxicants to the extent injurious to herself or others.  He resides in Pilot Rock, Ore.
Yamhill County
  • The Board accepted the voluntary surrender of Kristin R. Snyder’s Registered Nurse license (094006037RN) for violating the terms and conditions of a Board Order.  She resides in Newberg, Ore.
  • The Board suspended Deborah C. Carr’s Registered Nurse license (076009176RN) for 30 days for failing to take action to promote client safety, and assigning persons to perform functions for which they are not prepared or that are beyond their scope of duties.  She resides in Willamina, Ore.
Out of State
  • Kansas—The Board accepted the voluntary surrender of Sandi K. Brown’s Registered Nurse license (200642418RN) for failing to answer questions truthfully and failing to conform to the essential standards of nursing practice.  She resides in Overland Park, Kansas.
  • Washington—The Board revoked the Registered Nurse license of Bryan W. Corbitt (200140134RN), of Washougal, Wash., due to Child Pornography convictions.
  • Washington—The Board denied the RN application of Charles M. Herndon (099006852RN), of Vancouver, Wash., for violating the terms and conditions of a Board Order.
