Board of Nursing Disciplinary Actions Taken in July 2014
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 22 nurses or nursing assistants during its July 16, 2014, Board Meeting.

Clackamas County
  • The Board revoked the nursing assistant certificate of Nicole Hamberger (201012066CNA), of Milwaukie, Ore., due to Theft convictions, removing money from the workplace without authorization, and failing to cooperate with the Board during an investigation.
  • The Board suspended Mary L. M. Sule’s nursing assistant certificate (201010488CNA) for a minimum of 14-days for failing to cooperate with the Board during an investigation.  She resides in Milwaukie, Ore.
Columbia County
  • The Board denied the CNA application of Andrew M. Weir for using intoxicants to the extent injurious to himself or others.  He resides in Saint Helens, Ore.
Coos County
  • The Board suspended Tracy L. Button’s nursing assistant certificate (200512716CNA) for a minimum of 14-days for failing to maintain tax compliance with the Oregon Department of Revenue per ORS 183.417(3).  She resides in Coos Bay, Ore.
Deschutes County
  • The Board reprimanded Angela C. Clement’s Registered Nurse license (099006247RN) with conditions for the unauthorized removal of drugs and supplies from the workplace and performing acts beyond her authorized scope.  She resides in Bend, Ore.
  • The Board accepted the Voluntary Surrender of Shalonda A. McColm’s Registered Nurse license (200541073RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Bend, Ore.
Douglas County
  • The Board accepted the Voluntary Withdrawal of Jennifer M. Dew’s CNA Application due to a conviction for methamphetamine possession and use of intoxicants to the extent injurious to herself or others.  She resides in Roseburg, Ore.
  • The Board denied the CNA application of Carla S. Neith (201112902CNA) for exploiting the client relationship for personal gain and failing to answer questions truthfully.  She resides in Sutherlin, Ore.
  • The Board suspended Virginia L. Rondel’s Registered Nurse license (077009921RN) for a minimum of 14-days for failing to cooperate with the Board during an investigation.  She resides in Roseburg, Ore.
Jackson County
  • The Board placed Kendra D. Dengler’s practical nurse license (201230021LPN) on probation for 24-months for falsifying a client record, using intoxicants to the extent injurious to herself or others, and failing to answer questions truthfully.  She resides in Central Point, Ore.
Lane County
  • The Board suspended Gregory L. Ivy’s Registered Nurse license (080044735RN) for one year, to be followed by 36-months of probation, for the unauthorized removal of drugs from the workplace, practicing nursing while impaired, and using intoxicants to the extent injurious to himself or others.  He resides in Eugene, Ore.
  • The Board reprimanded Erin L. Bressert’s practical nurse license (201130281LPN) with conditions for failing to take action to preserve client safety, failing to maintain client records in a timely manner, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Eugene, Ore.
Linn County
  • The Board accepted the Voluntary Surrender of Joy L. Strasser’s Registered Nurse license (092006185RN) for violating the terms and conditions of a Board Order.  She resides in Sweet Home, Ore.
Multnomah County
  • The Board suspended Reola H. Box’s nursing assistant certificate (000020875CNA) for 15-days for failing to respect client rights and dignity and engaging in other unacceptable behavior in the presence of a client.  She resides in Portland, Ore.
  • The Board accepted the Voluntary Surrender of Stephen J. Emanuel’s Registered Nurse license (201142412RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  He resides in Portland, Ore.
  • The Board accepted the Voluntary Surrender of Wendy T. Noreen’s Registered Nurse license and Adult Nurse Practitioner certificate (091000506RN/091000506N3) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Portland, Ore.
  • The Board reprimanded Kakieba S. Taylor’s nursing assistant certificate (000038519CNA) with conditions for failing to respect client rights and engaging in other unacceptable behavior in the presence of a client.  She resides in Portland, Ore.
Umatilla County
  • The Board revoked the nursing assistant certificate of Cherita M. Waters (000043766CNA), of Pendleton, Ore., for violating the terms and conditions of a Board Order.
Washington County
  • The Board reprimanded Laurie K. McCain’s Registered Nurse license (092000147RN) for implementing standards of practice that jeopardized patient safety and failing to communicate client status information to members of the health care team.  She resides in Beaverton, Ore.
Out of State
  • Virginia—The Board denied the RN application of Betelehem M. Woldemariam for failing to meet licensure requirements.  She resides in Alexandria, Va.
  • Washington—The Board accepted the Voluntary Surrender of Jennifer E. Ayars’s Registered Nurse license (200942209RN) for unauthorized removal of drugs from the workplace and implementing nursing practice that jeopardized patient safety.  She resides in Vancouver, Wash.
  • Washington—The Board placed Tiffany M. Zimmerman’s Registered Nurse license (200743178RN) on probation for 36-months for inaccurate and incomplete recordkeeping, unauthorized removal of drugs from the workplace, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Vancouver, Wash.
