Board of Nursing Disciplinary Actions Taken in July 2017
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 32 nurses or nursing assistants during its July 12, 2017, Board Meeting.

Baker County
  • The Board suspended the nursing assistant certificate of Alicia D. Wirth (201500391CNA) for 90 days for failing to take action to promote a person’s safety, jeopardizing the safety of a person.  She resides in Baker City, Ore.
 
Clackamas County
  • The Board reprimanded the registered nurse license of Blaine J. Boyd (200541070RN), of Milwaukie, Ore., for violating the client’s rights to privacy.
  • The Board reprimanded the registered nurse license of Aimee S. Price (201393442RN), of Estacada, Ore., for inaccurate recordkeeping, unauthorized removal of supplies from the workplace, and failing to conform to the essential standards of acceptable nursing practice.
  • The Board placed the nursing assistant certificate of Cecilia M. Valencia (201705314CNA) on probation for 24 months for using intoxicants to the extent or in a manner injurious to herself or others.  She resides in Portland, Ore.
 
Coos County
  • The Board placed the nursing assistant and medication aide certificates of Sherri L. Grace Crawford (200310388CNA/201507452CMA) on probation for 24 months with conditions for demonstrated incidents of dishonesty, falsifying data, documenting services that were not provided, and jeopardizing the safety of a person under her care.  She resides in North Bend, Ore.
  • The Board placed the practical nurse license of Sandra M. Inman (200241484RN) on probation for 24 months for using intoxicants to the extent or in a manner dangerous or injurious to herself or others.  She resides in Coos Bay, Ore.
  • The Board placed the practical nurse license of Rochelle J. Russell (201391734LPN) on probation for 24 months for implementing standards of care that jeopardize patient safety.  She resides in Coos Bay, Ore.
 
Crook County
  • The Board reprimanded the nursing assistant certificate of Dawnielle M. Vaca (200911369CNA), of Prineville, Ore., for violating the client’s rights to privacy.
 
Deschutes County
  • The Board reprimanded the registered nurse license of Jeanne L. Hampton (201041563RN), of Bend, Ore., with conditions for violating the client’s rights to privacy.
  • The Board placed the nursing assistant certificate of April F. Jepsen (000021348CNA) on probation for 12 months for a conviction that relates to CNA duties.  She resides in Redmond, Ore.
  • The Board suspended the psychiatric mental health nurse practitioner certificate of Donald S. McFerran (081046617N6) for 14 days, followed by one-year probation for his PMHNP certificate and registered nurse license (081046617RN), for prescribing drugs in an unsafe manner, failure to properly assess client and document assessment when prescribing, and failing to conform to the essential standards of acceptable nursing practice.  He resides in Bend, Ore.
 
Douglas County
  • The Board revoked the registered nurse license of James N. Call (200841352RN), of Roseburg, Ore., for failing to comply with the terms and conditions of the Health Professionals’ Services Program.
  • The Board placed the registered nurse license of Neoma J. Keeler (200340207RN) on probation for 24 months for using intoxicants to an extent or in a manner dangerous or injurious to herself or others.  She resides in Roseburg, Ore.
  • The Board suspended the registered nurse license of Andrea R. Loveday (200942221RN) for one year, followed by one year of probation, for inaccurate and incomplete recordkeeping, leaving a nursing assistant assignment without properly notifying supervisory personnel, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Roseburg, Ore.
 
Lane County
  • The Board suspended the nursing assistant certificate of Rosanna J. Hoskins (201394446CNA) for 14 days for demonstrated incidents of reckless behavior, jeopardizing the safety of a person under the CNA’s care, and using intoxicants to the extent or in a manner injurious to herself or others.  She resides in Eugene, Ore.
 
Lincoln County
  • The Board denied the LPN application of A. Lyce A. Ruberg, of Yachats, Ore., for convictions that bear a demonstrable relationship to nursing, and failing to answer questions truthfully.
 
Linn County
  • The Board imposed a $500 civil penalty against Kimberly D. L. Madden's practical nurse license (201030193LPN) for practicing nursing without a current license.
 
Marion County
  • The Board reprimanded the registered nurse license of Robert E. Lucid (200841293RN), of Keizer, Ore., for inaccurate and incomplete recordkeeping, and failing to conform to the essential standards of acceptable nursing practice.
  • The Board revoked the nursing assistant certificate of Sierra R. Ortega (201310645CNA), of Salem, Ore., for violating the terms and conditions of a Board Order.
 
Multnomah County
  • The Board revoked the nursing assistant certificate of Ka D. Kunga (201111823CNA), of Portland, Ore., for abusing a person, jeopardizing the safety of a person in her care, and failing to respect client dignity and rights.
  • The Board reprimanded the practical nurse license of Jacob M. Oskar (201504153LPN), of Portland, Ore., for failing to take action to promote the client’s safety based on nursing assessment and judgment, and failing to conform to the essential standards of acceptable nursing practice.
 
Polk County
  • The Board accepted the Voluntary Withdrawal of Kathryn Alvarez’s CNA application for failing to meet certification requirements.  She resides in Independence, Ore.
  • The Board revoked the registered nurse license of Anna C. Jenik-Schnedler (201404336RN), of Monmouth, Ore., for failing to respect client dignity, and violating the client’s privacy.
 
Union County
  • The Board reprimanded the practical nurse license of Jacob Adams (201507606LPN), of La Grande, Ore., for failing to take action to promote client safety, failing to follow through with the plan of care, and failing to conform to the essential standards of acceptable nursing practice.
 
Washington County
  • The Board accepted the Voluntary Surrender of Carol Bennett’s registered nurse license (098007045RN) for failing to follow through with the plan of care, failing to administer medications in a manner consistent with state and federal law.  She resides in Beaverton, Ore.
  • The Board accepted the Voluntary Surrender of Karen C. Hansen’s practical nurse license (201501572LPN) for client abuse and neglect, unauthorized removal of drugs from the workplace, and using intoxicants to the extent injurious to herself or others.  She resides in Hillsboro, Ore.
  • The Board reprimanded the registered nurse license of Marcia L. Bogert (091006251RN), of Beaverton, Ore., for violating the client’s rights to privacy.
  • The Board reprimanded the registered nurse license of Mary Beth Stuver (200441940RN), of Portland, Ore., for violating the client’s rights to privacy.
 
Out of State
  • Arizona—The Board denied the RN and PMHNP application of Laura L. Smith (aka Sukanlaya Hughes), of Mesa, Ariz., for fraud during the licensure process, failing to answer questions truthfully, and failing to cooperate during the course of an investigation.
  • Washington—The Board denied the CNA application of Tammy S. Feller, of Glenwood, Wash., for using intoxicants to an extent or in a manner injurious to herself or others, and failing to answer questions truthfully.
  • Washington—The Board revoked the registered nurse license of Deanna L. McShane (201403709RN), of Vancouver, Wash., for failing to comply with the terms and conditions of the Health Professionals’ Services Program.
  • Washington—The Board denied the RN application of Wallace D. Small, of Seattle, Wash., for the unauthorized removal of supplies from the workplace, failing to answer questions truthfully, and failing to cooperate during the course of an investigation.
