Clackamas County

  • The Board revoked the nursing assistant certificate of Christina L. Harris (200913016CNA), of Canby, Ore., for violating the terms and conditions of a Board Order.
  • The Board revoked the nursing assistant certificate of Candice A. Haase (200112919CNA), of Wilsonville, Ore., for using intoxicants to the extent injurious to herself or others, and due to a conviction for Unlawful Possession of Methamphetamine.
  • The Board placed Lorri E. Castle’s Registered Nurse license (097000338RN) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Lake Oswego, Ore.

Coos County

  • The Board suspended Joshua A. Jennings’ nursing assistant certificate (200910518CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  He resides in North Bend, Ore.
  • The Board revoked the nursing assistant certificate of James E. Carlson (200312618CNA), of Coquille, Ore., for using intoxicants to the extent injurious to himself or others, and a mental condition that prevents him from performing CNA duties safely.

Deschutes County

  • The Board accepted the voluntary surrender of Constance C. Cronin’s Registered Nurse license (083043108RN) for violating the terms and conditions of the Health Professionals’ Services Program.  She resides in Bend, Ore.

Douglas County

  • The Board placed Diana M. Young’s Registered Nurse license (099000607RN) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Roseburg, Ore.

Jackson County

  • The Board accepted the voluntary surrender of Nicole Robustelli’s Registered Nurse license (200141391RN) for violating the terms and conditions of a Board Order.  She resides in Medford, Ore.
  • The Board revoked the Registered Nurse license of Andrew F. Schleinitz (201140066RN), of Medford, Ore., for practicing nursing while impaired, using intoxicants to the extent injurious to himself or others, and failing to answer questions truthfully.
  • The Board accepted the voluntary withdrawal of Sharon L. Brock’s CNA application for using intoxicants to the extent injurious to herself or others.  She resides in Phoenix, Ore.

Lane County

  • The Board revoked the nursing assistant certificate of Johnnie L. Mayer (200710421CNA), of McKenzie Bridge, Ore., for violating the terms and conditions of a Board Order.
  • The Board accepted the voluntary surrender of Lisa J. Anderson-Neel’s Registered Nurse license (081001010RN) due to an Identity Theft conviction, attempting to obtain unauthorized prescription drugs, failing to conform to the essential standards of acceptable nursing practice.  She resides in Eugene, Ore.
  • The Board revoked the nursing assistant certificate of Alice A. Andersen (200911059CNA), of Eugene, Ore., for violating the terms and conditions of a Board Order.
  • The Board reprimanded the nursing assistant certificate of Sean M. Wisdom (200711758CNA) for misrepresentation during the certification process and failing to answer questions truthfully.  He resides in Springfield, Ore.
  • The Board placed Debbie L. Jorgensen’s practical nurse license (201230296LPN) on probation for 24-months for a DUII conviction.  She resides in Dexter, Ore.
  • The Board placed Kathryn J. Cronin’s Registered Nurse license (094003269RN) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Eugene, Ore.
  • The Board denied the CNA application of Darryl J. Williams II, of Eugene, Ore., for failing to answer questions truthfully during the certification process.

Linn County

  • The Board accepted the voluntary surrender of Karen K. Thompson’s nursing assistant certificate (000018019CNA) for failing to perform CNA duties competently.  She resides in Albany, Ore.
  • The Board suspended Gerald A. McIntosh’s Registered Nurse license (094006413RN) for 30 days for implementing standards of care that jeopardize patient safety, and incomplete recordkeeping.  He resides in Albany, Ore.

Marion County

  • The Board accepted the voluntary surrender of Kimberly Wasson’s Registered Nurse license (000037370RN) for inaccurate recordkeeping, falsifying a client record, and using intoxicants to the extent injurious to herself or others.  She resides in Silverton, Ore.
  • The Board accepted the voluntary surrender of Abel C. Wenner’s nursing assistant certificate (200010262CNA) for an Assault conviction and for using intoxicants to the extent injurious to himself or others.  He resides in Salem, Ore.
  • The Board revoked the Registered Nurse license of Brendon L. Smith (200842510RN), of Salem, Ore., for violating the terms and conditions of the Health Professionals’ Services Program.

Multnomah County

  • The Board accepted the voluntary surrender of Carla M. Couch’s nursing assistant and medication aide certificates (000006529CNA/000006529CMA) for client neglect, performing acts beyond authorized duties, failing to administer medications as ordered, and failing to perform duties competently.  She resides in Portland, Ore.
  • The Board accepted the voluntary surrender of Barbara J. Watson’s practical nurse license (000003189LPN) for inaccurate and incomplete recordkeeping, failing to take action to promote client safety, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Portland, Ore.
  • The Board suspended Romana Sanchez-Vera’s nursing assistant certificate (200910654CNA) for 30 days for failing to respect client rights and dignity.  She resides in Portland, Ore.
  • The Board revoked the Registered Nurse license of Jimmie R. Bolin (200340317RN), of Corbett, Ore., for violating the terms and conditions of the Health Professionals’ Services Program.
  • The Board revoked the Registered Nurse license of Sarah J. Cogswell (096007082RN), of Portland, Ore., for failing to conform to the essential standards of acceptable nursing practice, and failing to cooperate with the Board during an investigation.
  • The Board revoked the nursing assistant certificate of Patience B. Kukena (200810804CNA), of Portland, Ore., due to a Theft conviction, and for failing to answer questions truthfully.
  • The Board reprimanded the nursing assistant certificate of Desiree L. Jones (000006714CNA) for leaving a CNA assignment without properly notifying an appropriate supervisor.  She resides in Portland, Ore.

Washington County

  • The Board revoked the nursing assistant certificate of Sara L. White (200312894CNA), of Beaverton, Ore., for using intoxicants to the extent injurious to herself or others, leaving a CNA assignment without properly notifying appropriate supervisors, and failing to cooperate with the Board during an investigation.
  • The Board placed an emergency suspension on Joan M. Alfano’s Registered Nurse license (200442134RN) for using intoxicants to the extent injurious to herself or others, failing to answer questions truthfully, and failing to cooperate with the Board during an investigation.  She resides in Portland, Ore.

Wheeler County

  • The Board suspended Pamela N. Helms-Penny’s nursing assistant certificate (201010891CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  She resides in Fossil, Ore.

Yamhill County

  • The Board reprimanded the nursing assistant certificate of Sharon J. Lane (200210603CNA) for a failing to perform CNA duties competently.  She resides in Sheridan, Ore.

Out of State

  • Florida— The Board reprimanded the Registered Nurse license of Stephanie White (201143351RN) with conditions for inaccurate and incomplete recordkeeping, and failing to maintain client records in a timely manner.  She resides in Largo, Fla.
  • Washington—The Board suspended Daphne E. Anderson’s Registered Nurse license (200540925RN) for 90 days, for neglecting a client, failing to communicate client status to members of the health care team, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Vancouver, Wash.
  • Idaho—The Board accepted the voluntary surrender of Corina J. Anderson’s Registered Nurse license (200141996RN) for violating the terms and conditions of the Health Professionals’ Services Program.  She resides in Payette, Idaho.
  • California— The Board accepted the voluntary surrender of Leslie T. Blasewitz’s Registered Nurse license (200441879RN) for violating the terms and conditions of a Board Order.  She resides in Susanville, Calif.
