Board of Nursing Disciplinary Actions Taken in June 2013

Baker County
  • The Board revoked the Registered Nurse license of Jimmie K. Hilderbrand (093000617RN), of Halfway, Ore., for failing to take action to promote client safety, failing to respect client dignity, using the client relationship for personal gain, inaccurate recordkeeping, and failing to conform to the essential standards of acceptable nursing practice.
Clackamas County
  • The Board accepted the voluntary surrender of Rebecca R. Smith’s nursing assistant certificate (201011778CNA) for using intoxicants to the extent injurious to herself or others and violating the terms and conditions of a Board Order.  She resides in Sandy, Ore.
  • The Board reprimanded Kimberly K. Clovis’s Registered Nurse license (200141296RN) for improperly delegating nursing tasks to unlicensed persons, and failing to supervise unlicensed persons to whom nursing tasks have been delegated.  She resides in Molalla, Ore.
Columbia County
  • The Board suspended Angela A. Rose’s nursing assistant certificate (200913198CNA) indefinitely for failing to cooperate with the Board during an investigation.  She resides in Warren, Ore.
Coos County
  • The Board reprimanded Devyn Phillip’s medication aide certificate (200920084CMA) for incomplete recordkeeping, failing to administer medications as ordered, and failing to perform CMA duties competently.  He resides in North Bend, Ore.
Deschutes County
  • The Board placed Sarah A. Little’s nursing assistant certificate (200212724CNA) on probation for 24-months for failing to perform CNA duties competently, exploiting the client relationship for personal gain, and failing to cooperate with the Board during an investigation.  She resides in Redmond, Ore.
Douglas County
  • The Board revoked the nursing assistant certificate of Misty C. Cheeseman (201112267CNA), of Riddle, Ore., for failing to perform CNA duties competently and failing to cooperate with the Board during an investigation.
Grant County
  • The Board placed Lindsay P. Madden’s Registered Nurse license (098000522RN) on probation for 24-months for obtaining unauthorized prescription drugs.  She resides in John Day, Ore.
Jackson County
  • The Board placed Thomas L. Weeks’s nursing assistant certificate (200710645CNA) on probation for 24-months for using intoxicants to the extent injurious to himself or others.  He resides in Medford, Ore.
  • The Board placed Karen L. Moore’s practical nurse license (201030021LPN) on probation for 24-months for failing to take action to preserve client safety, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Medford, Ore.
Lane County
  • The Board suspended Sandy M. Tarpley’s nursing assistant certificate (000034467CNA) for 30 days with conditions for failing to perform CNA duties competently.  She resides in Junction City, Ore.
  • The Board placed Spencer W. Saxe’s nursing assistant certificate (201011222CNA) on probation for 24-months for using intoxicants to the extent injurious to himself or others.  He resides in Eugene, Ore.
  • The Board suspended Karrissa J. Plaza’s nursing assistant certificate (200910757CNA) indefinitely for failing to cooperate with the Board during an investigation.  She resides in Eugene, Ore.
  • The Board revoked the nursing assistant certificate of Michael W. Kasahun (201010579CNA), of Eugene, Ore., for failing to answer questions truthfully.
  • The Board reprimanded Darrell J. Barber’s practical nurse license (200530314LPN) for failing to implement the plan of care, and failing to administer medications in a manner consistent with state law.  He resides in Eugene, Ore.
  • The Board reprimanded Peggy L. Barber’s Registered Nurse license (200540366RN) for failing to implement the plan of care, and failing to administer medications in a manner consistent with state law.  She resides in Eugene, Ore.
Linn County
  • The Board placed Amber L. Ingraham’s Registered Nurse license (200942928RN) on probation for 12-months for failing to conform to the essential standards of acceptable nursing practice.  She resides in Albany, Ore.
Morrow County
  • The Board suspended Jennifer E. Quick’s Registered Nurse license (200141549RN) for 30 days for administering unauthorized drugs, inaccurate recordkeeping, falsifying an agency record, and performing acts beyond her authorized scope.  She resides in Irrigon, Ore.
Multnomah County
  • The Board placed Rebecca S. Parker’s Registered Nurse license and Family Nurse Practitioner and Nurse Midwife Nurse Practitioner certificate (091007142RN/200150132NP/091007142N5) on probation for 24-months for failing to take action to preserve client safety, failing to communicate client status information to the health care team, inaccurate and incomplete recordkeeping, failing to provide requested documents, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Portland, Ore.
  • The Board placed Kari K. McKinley’s Registered Nurse license (200743411RN) on probation for 24-months for failing to comply with the requirements of the Nurse Monitoring Program.  She resides in Portland, Ore.
  • The Board suspended David T. Thomas’s nursing assistant certificate (200911941CNA) indefinitely for failing to cooperate with the Board during an investigation.  He resides in Portland, Ore.
  • The Board reprimanded Erica D. Speck’s practical nurse license (201030261LPN) for incomplete recordkeeping, and failing to communicate information about client status to the health care team.  She resides in Troutdale, Ore.
  • The Board placed Kesa E. Nomiyama’s Registered Nurse license (200242778RN) on probation for 36-months for using intoxicants to the extent injurious to herself or others, inaccurate recordkeeping, and failing to answer questions truthfully.  She resides in Portland, Ore.
  • The Board revoked the Registered Nurse license of Mary E. Harrison (091007196RN), of Portland, Ore., for violating the terms and conditions of a Board Order.
  • The Board accepted the voluntary surrender of Kaileene Gomez’s medication aide certificate (201020149CMA)for failing to document medications as administered, and failing to conform to the standards and authorized duties of a CMA.  She resides in Gresham, Ore.
  • The Board suspended Christina M. Armstrong’s nursing assistant certificate (000016179CNA) indefinitely for failing to cooperate with the Board during an investigation.  She resides in Fairview, Ore.
Washington County
  • The Board denied the LPN application of Tracy L. Massey, of Beaverton, Ore., for using intoxicants to the extent injurious to herself or others, and failing to cooperate during the course of an investigation.
  • The Board placed Angela Casella’s Registered Nurse license (086000404RN) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Gaston, Ore.
  • The Board revoked the Registered Nurse license of Barbara I. Teatro (097000734RN), of Portland, Ore., for violating the terms and conditions of the Health Professionals’ Services Program.
  • The Board revoked the Registered Nurse license of Maura A. Banse-Fay (095006629RN), of Tualatin, Ore., for obtaining unauthorized prescription drugs, inaccurate and incomplete recordkeeping, failing to cooperate with the Board during an investigation, and failing to conform to the essential standards of acceptable nursing practice.
Yamhill County
  • The Board suspended Kimberly Haas’s Registered Nurse license (084057114RN) indefinitely for failing to cooperate with the Board during an investigation.  She resides in McMinnville, Ore.
Out of State
  • Washington—The Board suspended Brenda L. Hanson’s Registered Nurse license (098006319RN) for 30 days, to be followed by 36 months of probation, for unauthorized removal of drugs from the workplace, and possessing unauthorized drugs.  She resides in Vancouver, Wash.
