Board of Nursing Disciplinary Actions Taken in June 2017
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 24 nurses or nursing assistants during its June 14, 2017, Board Meeting.

Clackamas County
  • The Board accepted the Voluntary Surrender of Jeannie L. Haak’s registered nurse license (200743854RN) for violating the terms of the HPSP monitoring agreement.  She resides in Oregon City, Ore.
Coos County
  • The Board reprimanded the registered nurse license of Kelle M. Takis (201241881RN), of Coos Bay, Ore., for violating the client’s rights to privacy.
Curry County
  • The Board placed the registered nurse license of Isaac S. Hodges (200941854RN) on probation for 24 months for exploiting the nurse-client relationship for personal gain, the unauthorized removal of drugs from the workplace, and possessing unauthorized drugs.  He resides in Brookings, Ore.
Douglas County
  • The Board accepted the Voluntary Surrender of Andrew B. Smith’s practical nurse license (200630269LPN) for failing to respect client rights and dignity, and performing acts beyond his authorized scope.  He resides in Roseburg, Ore.
Jackson County
  • The Board revoked the nursing assistant certificate of Maria Melendez-Rangel (201506549CNA), of Medford, Ore., for abusing a person, demonstrated incidents of abusive behavior, and failing to respect client dignity and rights.
Josephine County
  • The Board reprimanded the nursing assistant certificate of Stephanie A. Togstad (201602709CNA), of Cave Junction, Ore., with conditions for failing to violating a person’s rights to privacy, failing to maintain professional boundaries, and failing to conform to the essential standards of acceptable CNA performance.
Klamath County
  • The Board denied the CNA application of Tracey D. Wheeler, of Klamath Falls, Ore., for demonstrated incidents of dishonest behavior and failing to answer questions truthfully.
Lake County
  • The Board reprimanded the nursing assistant certificate of Charles P. Caffrey (201394199CNA), of Lakeview, Ore., for failing to implement the plan of care developed by the RN.
Lane County
  • The Board accepted the Voluntary Surrender of Aaron R. Boyce’s practical nurse license (200130150LPN) for violating the terms and conditions of a Board Order.  He resides in Eugene, Ore.
  • The Board suspended the nursing assistant certificate of Tonya L. Claxton (000025254CNA) for 14 days for leaving a nursing assistant assignment without properly notifying supervisory personnel.  She resides in Eugene, Ore.
  • The Board suspended the nursing assistant certificate of Suzanne R. Palese (201012847CNA) for 14 days with conditions, followed by 12 months of probation, for neglecting a person, falsifying data, failing to implement the plan of care developed by the RN.  She resides in Creswell, Ore.
  • The Board suspended the registered nurse license and family nurse practitioner certificate of Angela S. Zacharek (096000380RN/200650156NP) for 30 days, followed by 12 months of probation, for failing to dispense medication in a manner consistent with law, failing to maintain client records in a timely manner, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Eugene, Ore.
Marion County
  • The Board reprimanded the nursing assistant certificate of Rachel A. Jones (200812590CNA), of Salem, Ore., with conditions for demonstrated incidents of dishonesty, failing to implement the plan of care developed by the RN, failing to respect a client’s dignity, and violating the client’s rights to privacy.
  • The Board revoked the nursing assistant and medication aide certificates of Rochelle A. Kiluwe (000026678CNA/200020005CMA), of Salem, Ore., for verbally abusing a client, failing to document the administration of medications, and failing to cooperate during the course of an investigation.
Multnomah County
  • The Board accepted the Voluntary Surrender of Eric D. Colling’s registered nurse license (095000552RN) for practicing nursing while impaired and using intoxicants to the extent or in a manner injurious to himself or others.  He resides in Gresham, Ore.
  • The Board placed the registered nurse license of Laura F. Dunn (200341401RN) on probation for 24 months for using intoxicants to the extent or in a manner injurious to herself or others.  She resides in Portland, Ore.
  • The Board accepted the Voluntary Surrender of Randi L. Salinas’s nursing assistant and medication aide certificates (200912432CNA/201020094CMA) for failing to document medications as administered, and failing to administer medications as ordered by a LIP.  She resides in Portland, Ore.
Tillamook County
  • The Board suspended the nursing assistant certificate of Dee A. Johnson (201212865CNA) for 14 days for failing to follow the plan of care, and leaving a care assignment without properly notifying supervisory personnel.  She resides in Tillamook, Ore.
Union County
  • The Board placed the registered nurse license of DeeAnna Thompson (200743314RN) on probation for 24 months for using intoxicants to the extent or in a manner injurious to herself or others.  She resides in Union, Ore.
Washington County
  • The Board accepted the Voluntary Surrender of Kevin J. Michels’s registered nurse license (200440650RN) due to the voluntary surrender of his California license.  He resides in Portland, Ore.

Yamhill County

  • The Board suspended the nursing assistant certificate of Marc C. McPeters (201011135CNA) for a minimum of 14 days for engaging in abusive behavior towards a co-worker, and engaging in other unacceptable behavior in the presence of the client.  He resides in McMinnville, Ore.
Out of State
  • Arizona— The Board accepted the Voluntary Surrender of Doraine K. Lopex’s registered nurse license (201502237RN) for failing to follow through with the plan of care, incomplete and inaccurate recordkeeping, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Tucson, Ariz.
  • Florida—The Board accepted the Voluntary Withdrawal of Jessica M. Fackender’s RN application for supplying misleading information during the licensure process.  She resides in Spring Hill, Fla.
  • Washington— The Board revoked the nursing assistant certificate of Shelby R. Waddell (201500609CNA) for exploiting the client for personal gain and failing to cooperate with the Board during the course of an investigation. She resides in Connell, Wash.
