Board of Nursing Disciplinary Actions Taken in March 2013
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 23 nurses or nursing assistants during its March 20, 2013, Board Meeting.

Clackamas County
  • The Board accepted the voluntary surrender of Laura M. Alexander’s practical nurse license (090005276LPN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Boring, Ore.
  • The Board placed Maryanne Carlson’s Registered Nurse and limited licenses (200741731RN/L001141RN) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in West Linn, Ore.
  • Coos County
    The Board suspended Mary A. Gallino’s Registered Nurse license (082007544RN) for 60 days, for improperly delegating tasks of  nursing care, inaccurate and incomplete recordkeeping, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Bandon, Ore.
Curry County
  • The Board accepted the voluntary surrender of Mary L. Boyle’s practical nurse license (200830330LPN) for possessing prescription drugs without authorization.  She resides in Gold Beach, Ore.
    Hood River County
  • The Board accepted the voluntary surrender of Charles R. Christensen’s Registered Nurse license (200240806RN) for the unauthorized removal of drugs from the workplace, and falsifying agency records.  He resides in Hood River, Ore.
Josephine County
  • The Board suspended Linda L. Einsel’s nursing assistant certificate (200111552CNA) for 60 days, for failing to respect client rights, and engaging in other unacceptable behavior in the presence of a client.  She resides in Murphy, Ore.
  • The Board revoked the Registered Nurse license of Mikki A. Hanson (088007188RN), of Merlin, Ore., for violating the terms and conditions of a Board Order.
Klamath County
  • The Board revoked the practical nurse license of Elisa A. Hunt (200830077LPN), of Klamath Falls, Ore., for practicing nursing without a license, failing to provide documents requested by the Board, failing to cooperate with the Board during an investigation.
Lane County
  • The Board revoked the practical nurse license of Ellen K. Chiapuzio (088003130LPN), of Eugene, Ore., for client neglect, falsifying an agency record, using intoxicants to the extent injurious to herself or others, and failing to cooperate with the Board during an investigation.
Linn County
  • The Board revoked the nursing assistant certificate of Christina S.E. Schwend (200510719CNA), of Albany, Ore., for failing to cooperate with the Board during an investigation.
Marion County
  • The Board reprimanded Guadalupe Gutierrez’s nursing assistant certificate (200510455CNA) for using intoxicants to the extent injurious to herself or others.  She resides in Salem, Ore.
  • The Board reprimanded Martha S. Butler’s nursing assistant certificate (200910176CNA) for abusing a client.  She resides in Salem, Ore.
  • The Board placed Timothy S. Horner’s Registered Nurse license (200840881RN) on probation for 24-months for using intoxicants to the extent injurious to himself or others.  He resides in Salem, Ore.
Multnomah County
  • The Board accepted the voluntary surrender of Gayle M. Martell’s Registered Nurse license (095006868RN) for violating the terms and conditions of a Board Order.  She resides in Gresham, Ore.
  • The Board revoked the nursing assistant certificate of Laura O. Nimo (200610232CNA), of Portland, Ore., for violating the terms and conditions of a Board Order.
  • The Board revoked the Registered Nurse license of Kristopher K. Newport (201040181RN), of Portland, Ore., for using intoxicants to the extent injurious to himself or others, and failing to cooperate with the Board during an investigation.
  • The Board revoked the nursing assistant certificate of Jeffery T. Farrell (000040737CNA), of Portland, Ore., for exploiting clients for personal gain, removing drugs from the workplace without authorization, and using intoxicants to the extent injurious to himself or others.
  • The Board reprimanded Serena L. Watson’s Registered Nurse license (200940087RN) with conditions for failing to complete a nursing assignment without notification, altering an agency record, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Portland, Ore.
Washington County
  • The Board accepted the voluntary surrender of Kathryn W. Go Smith’s Registered Nurse license (097000356RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Portland, Ore.
  • The Board revoked the nursing assistant certificate of Yvonne M. McKinney (000036396CNA), of Cornelius, Ore., for violating the terms and conditions of a Board Order.
Out of State
  • California—The Board suspended Daniel J. Thomas’s Registered Nurse license (200340397RN) for 30 days, for client abuse and neglect, and failing to conform to the essential standards of acceptable nursing practice.  He resides in San Diego, Calif.
  • Washington—The Board accepted the voluntary surrender of Matthew M. Coleman’s practical nurse license (200530335LPN) for a Rape of a Child conviction.  He resides in Fort Lewis, Wash.
  • Washington—The Board accepted the voluntary surrender of Suzanne M. Ryan’s Registered Nurse license (201240173RN) for a license suspension in Kansas.  She resides in Hangzhou, China.
