Board of Nursing Disciplinary Actions Taken in March 2014

Clackamas County
  • The Board accepted the voluntary surrender of Jennifer L. Villanueva’s Registered Nurse license (096000465RN) for using intoxicants to the extent injurious to herself or others, practicing while impaired, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Gladstone, Ore.
  • The Board placed Edie A. Donnelly’s Registered Nurse license (200440840RN) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Beavercreek, Ore.
Clatsop County
  • The Board accepted the voluntary surrender of Cristen M. Peterson’s Registered Nurse license (201142642RN) for failing to maintain professional boundaries with a client, failing to answer questions truthfully, using intoxicants to the extent injurious to herself or others, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Astoria, Ore.
Crook County
  • The Board accepted the voluntary surrender of Patricia A. Scholz’s Registered Nurse license (000023800RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Prineville, Ore.
Josephine County
  • The Board revoked the Registered Nurse license of Donald W. Richie (201143073RN), of Cave Junction, Ore., for failing to provide requested documents, and failing to cooperate with the Board during an investigation.
Lane County
  • The Board reprimanded Margaret A. Leonard’s Women’s Health Care Nurse Practitioner certificate (000030867N7) for prescribing drugs to an individual who was not her patient.  She resides in Eugene, Ore.
  • The Board accepted the voluntary surrender of Judie C. Finlayson’s Registered Nurse license (090003155RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Eugene, Ore.
  • The Board placed Kristin E. Cutter’s Registered Nurse license (200541585RN) on probation for 24-months for inaccurate and incomplete recordkeeping, and using intoxicants to the extent injurious to herself or others.  She resides in Eugene, Ore.
  • The Board suspended Billie A. Cross’s nursing assistant certificate (000032395CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Eugene, Ore.
Marion County
  • The Board placed Jill A. Henschel’s Registered Nurse license (089003340RN) on probation for 36-months for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Woodburn, Ore.
Multnomah County
  • The Board accepted the voluntary surrender of Kathleen J. Phillips’s Registered Nurse license (081001871RN) for violating the terms and conditions of a Board Order.  She resides in Portland, Ore.
  • The Board revoked the nursing assistant certificate of Erick O. Murillo-Camacho (201012720CNA), of Portland, Ore., for violating the terms and conditions of a Board Order.
Polk County
  • The Board revoked the nursing assistant certificate of Patrick D. Smith (200910588CNA), of Rickreall, Ore., for exploiting the client relationship for personal gain.
  • The Board revoked the nursing assistant and medication aide certificates of Laurie L. Johnson (000019286CNA/000019286CMA), of Monmouth, Ore., for using unauthorized prescription drugs, and violating the terms and conditions of a Board Order.
Umatilla County
  • The Board revoked the Registered Nurse license of Megan A. Simmons (200441210RN), of Pendleton, Ore., for failing to comply with the terms and conditions of the Health Professionals’ Services Program.
Washington County
  • The Board suspended Christine D. Watson’s Registered Nurse license (200441351RN) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Tigard, Ore.
  • The Board revoked the nursing assistant certificate of James D. Shinholster (201110018CNA), of Portland, Ore., for exploiting the client relationship for personal gain, obtaining unauthorized drugs, failing to provide requested documents, and failing to cooperate with the Board during an investigation.
  • The Board suspended Christina F. DePaz’s nursing assistant certificate (201010801CNA) for 30 days for due to a Theft conviction and failing to cooperate with the Board during an investigation.  She resides in Beaverton, Ore.
Yamhill County
  • The Board accepted the voluntary surrender of Tammy Jean M. Holt’s nursing assistant certificate (200010450CNA) for using unauthorized prescription drugs.  She resides in Sheridan, Ore.
  • The Board placed Brett E. Hereford’s Registered Nurse license (200441071RN) on probation for 24-months for using intoxicants to the extent injurious to himself or others.  He resides in Newberg, Ore.
  • The Board reprimanded April L. Brown’s Registered Nurse license (200942306RN) for fraud and failing to answer questions truthfully.  She resides in Newberg, Ore.
Out of State
  • Missouri—The Board suspended William D. Brunk’s Registered Nurse license (201240077RN) for a minimum of 14 days for receiving discipline in another state.  He resides in Saint Louis, Mo.
  • Washington—The Board revoked the nursing assistant certificate of Bethany L. Austin (200712609CNA), of Dallesport, Wash., for exploiting the client for personal gain and a Criminal Mistreatment conviction.
  • Washington—The Board accepted the voluntary surrender of James W. Green’s Registered Nurse license (201243535RN) for receiving discipline in another state.  He resides in Vancouver, Wash.
