Board of Nursing Disciplinary Actions Taken in May 2013

Clackamas County
  • The Board reprimanded Monica Ferguson’s Family Nurse Practitioner certificate (200650008NP) for prescribing drugs to an individual who was not her client.  She resides in Clackamas, Ore.
  • The Board reprimanded Kimberly A. Babler’s nursing assistant certificate (200511528CNA) for inaccurate and incomplete recordkeeping, and failing to communicate information regarding patient’s status.  She resides in Happy Valley, Ore.
Coos County
  • The Board revoked the nursing assistant certificate of Joshua A. Jennings (200910518CNA), of North Bend, Ore., for violating the terms and conditions of a Board Order.
  • The Board reprimanded Joanne M. Rutland’s Psychiatric Mental Health Nurse Practitioner certificate (200150043NP) for prescribing drugs to an individual who was not her client.  She resides in Coos Bay, Ore.
Douglas County
  • The Board accepted the voluntary withdrawal of Laurel A. Harvey’s RN application for using intoxicants to the extent injurious to herself or others.  She resides in Roseburg, Ore.

Jackson County

  • The Board suspended Sean Sweeney’s nursing assistant certificate (201113211CNA) indefinitely for failing to cooperate with the Board during an investigation.  He resides in Eagle Point, Ore.
  • The Board denied Stephanie L. Phillip’s CNA application (200810323CNA) for failing to cooperate with the Board during an investigation and failing to answer questions truthfully.  She resides in Medford, Ore.
Linn County
  • The Board suspended Veronica N. Camarillo’s nursing assistant certificate (200111544CNA) indefinitely for failing to cooperate with the Board during an investigation.  She resides in Albany, Ore.
Tillamook County
  • The Board suspended Sasha M. Madrid’s nursing assistant certificate (201310336CNA) indefinitely for failing to cooperate with the Board during an investigation.  She resides in Rockaway Beach, Ore.
Yamhill County
  • The Board placed Lisa Holladay’s Registered Nurse license (084056631RN) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in McMinnville, Ore.
Out of State
  • Idaho—The Board denied Christian R. Ries’s CNA application for using intoxicants to the extent injurious to himself or others.  He resides in Payette, Idaho.
  • New York—The Board denied Justin Forth’s RN application for client abuse and failing to cooperate with the Board during an investigation.  He resides in Ogdensburg, N.Y.
  • Texas—The Board accepted the voluntary surrender of Tavia L. Johnson’s Registered Nurse license (200340632RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in North Richland Hills, Texas.
  • Washington—The Board reprimanded Evett A. Gradwohl’s Registered Nurse license (200540463RN) for engaging in other unacceptable behavior towards or in the presence of a client.  She resides in Vancouver, Wash.
  • Thailand— The Board revoked the Registered Nurse license of Patrick B. Rice (088003079RN) for falsifying an agency record, misrepresentation during the licensure process, and failing to answer questions truthfully.  He resides in Bangkok, Thailand.
