Board of Nursing Disciplinary Actions Taken in May 2014
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 15 nurses or nursing assistants during its May 14, 2014, Board Meeting.

Clackamas County
  • The Board denied the CNA and CMA applications of Tanra L. Gross (200112917CNA/201120052CMA), of Boring, Ore., for a Theft conviction, representing herself as a CNA without current CNA certification, and functioning as a medication assistant without current certification as a medication aide.
  • The Board reprimanded Paul R. Browning’s Registered Nurse license (200440335RN) with conditions for performing acts beyond his authorized scope, and failing to conform to the essential standards of acceptable nursing practice.  He resides in Canby, Ore.
Douglas County
  • The Board placed Kristi Caylor’s Registered Nurse license (200341209RN) on probation for 24-months for inaccurate recordkeeping, using intoxicants to the extent injurious to herself or others.  She resides in Roseburg, Ore.
Grant County
  • The Board revoked the nursing assistant certificate of Erinn C. Parsons (200211375CNA), of John Day, Ore., due to a conviction for False Claim of a Healthcare Payment.
Harney County
  • The Board reprimanded Nancy Carpentier’s Registered Nurse license (200541037RN) for altering a client record and performing acts beyond her authorized scope.  She resides in Hines, Ore.
Josephine County
  • The Board suspended Linda M. Picker-Johnson’s Registered Nurse license and Adult Nurse Practitioner certificate (200841022RN/200850056NP) with conditions, to be followed by 12-month probation, for failing to take action to preserve client safety based on nursing assessment and judgment, failing to properly assess and document client assessment when prescribing.  She resides in Grants Pass, Ore.
Multnomah County
  • The Board suspended Tana S. Wittwer’s Registered Nurse license (200540946RN) for failing to cooperate with the Board during an investigation.  She resides in Portland, Ore.
  • The Board revoked the Registered Nurse license of Jason R. Vargo (200242641RN), of Portland, Ore., for violating the terms and conditions of a Board Order and using intoxicants to the extent injurious to himself or others.
  • The Board accepted the withdrawal of Abdi A. Sherif’s CNA application due to a disciplinary action with the Oregon Board of Massage Therapists.  He resides in Portland, Ore.
  • The Board revoked the nursing assistant certificate of Cherie E. Page (000029992CNA), of Portland, Ore., due to a conviction for False Claim of a Healthcare Payment, and failing to cooperate with the Board during an investigation.
  • The Board suspended Dana A. Lightning’s practical nurse license (200930136LPN) for 30 days for assigning persons to perform functions for which they are not prepared, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Portland, Ore.
Washington County
  • The Board placed Cheryl R. Russo’s Registered Nurse license (200640046RN) on probation for 12-months for practicing nursing while impaired.  She resides in Hillsboro, Ore.
  • The Board suspended Jamie M. Kabelowsky’s Registered Nurse license (201341599RN) for failing to cooperate with the Board during an investigation.  She resides in Hillsboro, Ore.
  • The Board placed Gaylene Copley’s Registered Nurse license (201140622RN) on probation for 24-months for unauthorized removal of drugs from the workplace, failing to answer questions truthfully, and using intoxicants to the extent injurious to herself or others.  She resides in Hillsboro, Ore.
Yamhill County
  • The Board suspended Alexandria E. Parker’s nursing assistant certificate (200910045CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Newberg, Ore.
