Board of Nursing Disciplinary Actions Taken in May 2017
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 20 nurses or nursing assistants during its May 10, 2017, Board Meeting.

Benton County
  • The Board placed the nursing assistant certificate of Christina S. E. Schwend (200510719CNA) on probation for 24 months due to impairment.  She resides in Corvallis, Ore.

Clackamas County

  • The Board accepted the Voluntary Surrender of Esperanza M. De La Cruz’s nursing assistant certificate (200211621CNA) for violating the terms and conditions of a Board Order.  She resides in Milwaukie, Ore.
Deschutes County
  • The Board placed the registered nurse license of Maggie J. Ciomber (201703392RN) on probation for 24 months for the unauthorized removal of drugs from the workplace, falsifying agency records, and using intoxicants to the extent injurious or in a manner dangerous to herself or others.  She resides in Bend, Ore.
  • The Board suspended the registered nurse license of Roberta I. Garrett (200942921RN) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation.  She resides in Bend, Ore.
Lane County
  • The Board revoked the practical nurse license of Tiffany N. Garcia (200930268LPN), of Eugene, Ore., for implementing standards of nursing care that jeopardize patient safety, incomplete recordkeeping, failing to communicate information regarding client status to members of the health care team, and failing to answer questions truthfully.
  • The Board accepted the Voluntary Withdrawal of Jharumi Lopez’s CNA application for misrepresentation in applying for a certificate.  He resides in Springfield, Ore.
  • The Board suspended the nursing assistant certificate of Tabitha M. Price (201393975CNA) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation.  She resides in Springfield, Ore.
  • The Board accepted the Voluntary Surrender of LaDonna M. Quinn’s nursing assistant certificate (201405269CNA) for abuse, failing to respect client dignity, and engaging in unacceptable behavior towards clients.  She resides in Springfield, Ore.
  • The Board placed the practical nurse license of Katrina A. Ritchey (201408071LPN) on probation for 12 months for implementing standards of nursing care that jeopardize patient safety, performing acts beyond her authorized scope, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Eugene, Ore.
Linn County
  • The Board placed the practical nurse license of Michele L. McMullen (201330210LPN) on probation for 24 months for practicing nursing while impaired, and using intoxicants to the extent injurious or in a manner dangerous to herself or others.  She resides in Albany, Ore.
Multnomah County
  • The Board accepted the Voluntary Withdrawal of Christa R. Hallowell’s RN application (200341535RN) for using intoxicants to an extent or in a manner injurious to herself or others, and convictions for crimes bearing demonstrable relationship to nursing.  She resides in Fairview, Ore.
  • The Board revoked the nursing assistant certificate of Crystal E. Hill (200913462CNA), of Gresham, Ore., for demonstrated incidents of dishonesty, leaving a nursing assistant assignment without properly notifying appropriate supervisory personnel, and failing to answer questions truthfully.
  • The Board placed the registered nurse license of Thana M. Martin (088006000RN) on probation for 24 months for conduct derogatory to the standards of nursing.  She resides in Portland, Ore.
  • The Board revoked the nursing assistant certificate of Adeladilew A. Mekonen (201500736CNA), of Portland, Ore., for abusing a person, engaging in sexual misconduct with a client in any setting, engaging in sexual misconduct in the workplace, and failing to maintain professional boundaries.
  • The Board reprimanded the practical nursing license of Hanako B. Paul (098003038LPN), of Portland, Ore., with conditions for failing to dispense medications, and failing to conform to the essential standards of acceptable nursing practice.
  • The Board revoked the nursing assistant and medication aide certificates of Randi L. Salinas (200912432CNA/201020094CMA), of Portland, Ore., for failing to administer medications as ordered by a LIP, falsifying the medication administration record, demonstrated incidents of dishonesty, and jeopardizing the safety of a person under her care.
  • The Board placed the nursing assistant certificate of Jennifer S. Stalker (201703433CNA) on probation for 24 months for using intoxicants to the extent injurious or in a manner dangerous to herself or others.  She resides in Portland, Ore.
Umatilla County
  • The Board placed the practical nurse license of Joan M. Wilson (200130020LPN) on probation for 24 months for implementing standards of nursing care that jeopardize patient safety, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Milton-Freewater, Ore.
Out of State
  • Washington—The Board accepted the Voluntary Withdrawal of Kathi L. Doty’s RN application (201406588RN) for using intoxicants to an extent or in a manner injurious to herself or others.  She resides in Vancouver, Wash.
  • Washington—The Board placed the registered nurse license of Charles A. Henderson (200241980RN) on probation for 24 months for self-administering unauthorized drugs, and using intoxicants to the extent injurious or in a manner dangerous to himself or others.  He resides in Vancouver, Wash.
