Benton County
- The Board accepted the voluntary surrender of Rachel T. Alegado’s nursing assistant certificate (000023654CNA) for client neglect and failing to perform CNA duties competently. She resides in Corvallis, Ore.
Clackamas County
- The Board revoked the practical nurse license of Cynthia A. Christiansen (000006001LPN), of Milwaukie, Ore., for obtaining drugs without authorization, inaccurate and incomplete recordkeeping, and failing to conform to the essential standards of acceptable nursing practice.
- The Board placed Denise M. Hemenway-Mantei’s practical nurse license (200930526LPN) on probation for 24-months for implementing standards of nursing care that jeopardized patient safety, failing to take action to preserve patient safety, and failing to conform to the essential standards of acceptable and prevailing nursing practice. She resides in Eagle Creek, Ore.
- The Board accepted the voluntary surrender of Karen E. Shaw’s Adult Nurse Practitioner certificate (085074804N3) for prescribing drugs in an unsafe manner, failure to properly assess and document assessment when prescribing, developing standards of practice that jeopardize patient safety, and failing to conform to the essential standards of acceptable nursing practice. She resides in Oregon City, Ore.
Coos County
- The Board revoked the nursing assistant certificate of Patrick R. Bohannon (201111926CNA), of Bandon, Ore., due to a Abuse conviction and failing to cooperate with the Board during the course of an investigation.
Jackson County
- The Board suspended Jillian J. Hartman’s nursing assistant certificate (201113133CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation. She resides in Medford, Ore.
Josephine County
- The Board revoked the Registered Nurse license of Linda J. Pattinson (200441423RN), of Grants Pass, Ore., for unauthorized removal of drugs from the workplace, inaccurate recordkeeping, using intoxicants to the extent injurious to herself or others, and failing to conform to the essential standards of acceptable nursing practice.
Lane County
- The Board suspended and reprimanded Kevin E. Mishler’s Registered Nurse license (086003303RN) for practicing nursing without a current Oregon license. He resides in Fall Creek, Ore.
- The Board reprimanded Gina M. Sumpter’s Registered Nurse license (200142131RN) for the unauthorized removal of drugs from another person. She resides in Eugene, Ore.
- The Board accepted the voluntary surrender of Lucinda M. Trammell’s Registered Nurse license (200341114RN) for attempting to obtain unauthorized drugs, falsifying an agency record, and failing to conform to the essential standards of acceptable nursing practice. She resides in Springfield, Ore.
- The Board reprimanded Jeanne L. Wand’s Registered Nurse license (200743790RN) for failing to answer questions truthfully. She resides in Eugene, Ore.

- The Board suspended Ryan Penner’s nursing assistant certificate (201011724CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation. He resides in Salem, Ore.

- The Board accepted the voluntary surrender of Stephen M. Hopkins Registered Nurse license (079042568RN) for practicing while impaired and using intoxicants to the extent injurious to himself or others. He resides in Portland, Ore.
- The Board suspended Jennifer M. Sanchez’s nursing assistant certificate (200112863CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation. She resides in Portland, Ore.
Tillamook County
- The Board suspended and placed on probation Michele G. Vanderwaal’s Psychiatric Mental Health Nurse Practitioner certificate (200350142NP) for inaccurate and incomplete recordkeeping, failing to maintain client records in a timely manner, violating the terms and conditions of the Health Professionals’ Services Program, and failing to conform to the essential standards of acceptable nursing practice. - Washington—The Board reprimanded Blair V. Lower’s Registered Nurse license (201041601RN) for inaccurate and incomplete recordkeeping, and failing to maintain client records in a timely manner. She resides in Vancouver, Wash.
- Michigan—The Board suspended Bonnie J. Sheller’s nursing assistant certificate (201010049CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation. She resides in Grand Rapids, Mich.
- New Jersey—The Board denied the RN application of Valentina Taneska Coleska of Garfield, N.J., for failing to meet the educational requirements for licensure.
