Board of Nursing Disciplinary Actions Taken in November 2013

Benton County
  • The Board denied Daniel V. Marker’s nursing assistant application due to a conviction for a crime that bears a demonstrable relationship to CNA duties.  He resides in Corvallis, Ore.
Clackamas County
  • The Board revoked the Registered Nurse license of Martin P. Lavell (000030302RN), of Sandy, Ore., for failing to take action to preserve client safety, inaccurate and incomplete recordkeeping, failing to communicate information regarding client status to the healthcare team, and failing to conform to the essential standards of acceptable nursing practice.
  • The Board suspended Larry L. Stiff’s nursing assistant certificate (200911357CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  He resides in Oregon City, Ore.
  • The Board accepted the voluntary surrender of David E. Nolen’s Registered Nurse license (093000365RN) due to allegations that he purchased child pornography over the internet.  He resides in Happy Valley, Ore.
  • The Board suspended Kathleen A. Healy’s Registered Nurse license (000032439RN) for 30 days for client neglect, failing to take action to preserve client safety, incomplete recordkeeping, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Oak Grove, Ore.
  • The Board placed Patrick J. Hallinan’s nursing assistant certificate (200811056CNA) on probation for 24-months for using intoxicants to the extent injurious to himself or others.  He resides in Portland, Ore.
  • The Board placed Patricia M. Dolan’s nursing assistant certificate (201211570CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in West Linn, Ore.
Columbia County
  • The Board reprimanded Julie Ana M. Johnson’s nursing assistant certificate (201011647CNA) for convictions that bear a demonstrable relationship to the duties of a CNA, and using intoxicants to the extent injurious to herself or others.  She resides in Vernonia, Ore.
Coos County
  • The Board suspended Rachel S. D. Sullivan’s nursing assistant certificate (201210724CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Coos Bay, Ore.
  • The Board suspended Marcella S. Simerly’s Registered Nurse license (087003229RN) for 30 days for client abuse, leaving a care assignment without notifying the appropriate personnel, performing acts beyond her authorized scope, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Coquille, Ore.
Crook County
  • The Board accepted the voluntary surrender of Dawn R. Rardin-Bates’s Registered Nurse license (094000577RN) for violating the terms and conditions of the Health Professionals’ Services Program.  She resides in Prineville, Ore.
Douglas County
  • The Board revoked the Registered Nurse license of Louisa DeLelle Unicume (086003221RN), of Camas Valley, Ore., for practicing nursing while impaired, using intoxicants to the extent injurious to herself or others, and failing to cooperate with the Board during an investigation.
  • The Board accepted the voluntary surrender of Jennifer A. Himmelreich’s Registered Nurse license (200241958RN) for possessing unauthorized drugs and using intoxicants in a manner injurious to herself or others.  She resides in Sutherlin, Ore.
Jackson County
  • The Board suspended Simone Muro’s nursing assistant certificate (201012087CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Medford, Ore.
  • The Board accepted the voluntary surrender of Julie A. Crafts’ Registered Nurse license (099006934RN) for conviction of a crime that bears demonstrable relationship to the practice of nursing, practicing while impaired, and using intoxicants to the extent injurious to herself or others.  She resides in Medford, Ore.
Lane County
  • The Board accepted the voluntary surrender of Jessica A. Scharf-Bailey’s Registered Nurse license (200540345RN) for violating the terms and conditions of a Board Order.  She resides in Florence, Ore.
  • The Board placed Dorinda L. Schaper’s Registered Nurse license (095003006RN) on probation for 48-months for the unauthorized removal of drugs from the workplace, incomplete recordkeeping, practicing nursing while impaired, and using intoxicants to the extent injurious to herself or others.  She resides in Eugene, Ore.
  • The Board accepted the voluntary surrender of April M. Miotke’s practical nurse license and nursing assistant certificate (201130524LPN/000029769CNA) for unauthorized removal of drugs from the workplace, failing to communicate information regarding client status to the healthcare team.  She resides in Cottage Grove, Ore.
Linn County
  • The Board placed Randall B. Miller’s practical nurse license (200930309LPN) on probation for 12-months for the inaccurate and incomplete recordkeeping, implementing a standards of practice that jeopardized patient safety, and failing to conform to the essential standards of acceptable nursing practice.  He resides in Halsey, Ore.
  • The Board suspended Suzanne M. Ferrell’s Registered Nurse license (090000188RN) for 30 days for deceit in the practice of nursing and aiding an individual to circumvent a law, rule or regulation.  She resides in Albany, Ore.
Marion County
  • The Board accepted the voluntary surrender of Madonna C. Gentemann’s Registered Nurse license (200440728RN) for the unauthorized removal of drugs from the workplace and practicing while impaired.  She resides in Salem, Ore.
Multnomah County
  • The Board placed Jacob L. Klein’s Registered Nurse license (201042873RN) on probation for 24-months for the unauthorized removal of drugs from the workplace, practicing while impaired, and failing to conform to the essential standards of acceptable nursing practice.  He resides in Portland, Ore.
  • The Board revoked the Registered Nurse license and Women’s Health Care Nurse Practitioner certificate of Harmony H. Holt (200740575RN/201150156NP), of Portland, Ore., for failing to maintain professional boundaries with a client, exploiting the nurse-client relationship for personal gain, inaccurate and incomplete recordkeeping, and prescribing drugs to an individual not within her scope of practice.
  • The Board revoked the Registered Nurse license of Christy L. Durbin (088000178RN), of Portland, Ore., for furnishing unauthorized prescription medication and failing to answer questions truthfully.
  • The Board placed Alexandra F. Appleton’s nursing assistant certificate (201011915CNA) on probation for 24-months for convictions of crimes that bear a demonstrable relationship to the duties of a CNA, and using intoxicants to the extent injurious to herself or others.  She resides in Portland, Ore.
Union County
  • The Board placed Matthew S. Grove’s Registered Nurse license (200742451RN) on probation for 24-months for failing to answer questions truthfully, failing to conform to the essential standards of acceptable nursing practice, and using intoxicants to the extent injurious to himself or others.  He resides in La Grande, Ore.
Washington County
  • The Board placed Marie A. Valente’s Registered Nurse license (094000457RN) on probation for 24-months for possessing unauthorized drugs, and using intoxicants to the extent injurious to herself or others.  She resides in Beaverton, Ore.
  • The Board revoked the nursing assistant certificate of Thomas Kangar (201011120CNA), of Portland, Ore., for a conviction of a crime that bears a demonstrable relationship to the duties of a CNA.

Yamhill County

  • The Board placed Debra L. Schwartzkopff’s Registered Nurse license (094006259RN) on probation for 24-months for practicing while unable to perform due to a mental impairment.  She resides in Newberg, Ore.
Out of State
  • Washington—The Board reprimanded Audrey Q. Adames’s practical nurse license (201030059LPN) with conditions for inaccurate recordkeeping and failing to maintain client records in a timely manner.  She resides in Vancouver, Wash.
  • Washington—The Board suspended Melanie G. Casebolt’s Registered Nurse license (081001797RN) for 30 days with conditions for client neglect and failing to take action to preserve client safety.  She resides in Camas, Wash.
  • Washington—The Board suspended Dawn M. Frazier’s Registered Nurse license (096006696RN) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Vancouver, Wash.
  • Washington— The Board accepted the voluntary surrender of Brenda L. Hanson’s Registered Nurse license (098006319RN) for the unauthorized removal of drugs from the workplace.  She resides in Vancouver, Wash.
  • Washington—The Board placed Shawna E. Hebert’s Registered Nurse license (200640222RN) on probation for 24-months for the unauthorized removal of drugs from the workplace and practicing while impaired.  She resides in Vancouver, Wash.
  • Pennsylvania—The Board reprimanded Sandy J. Locker’s Registered Nurse license (200743703RN) with conditions for client neglect, failing to take action to preserve client safety, and incomplete recordkeeping.  She resides in Montgomery, Pa.
  • Washington—The Board suspended Tina M. Morrison’s Registered Nurse license (200740031RN) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Greenacres, Wash.
  • Washington—The Board accepted the voluntary surrender of Retta A. Vantrease’s Registered Nurse license (081001722RN) for incomplete recordkeeping, failing to take action to preserve client safety, and gross incompetence.  She resides in Vancouver, Wash.
  • New York—The Board denied the RN application of Dawn L. Osborne-Watts, of Brooklyn, N.Y., for misrepresentation during the licensure process and failing to answer questions truthfully.
