Board of Nursing Disciplinary Actions Taken in November 2016
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 27 nurses or nursing assistants during its November 16, 2016, Board Meeting.

Clackamas County
  • The Board suspended the registered nurse license and family nurse practitioner certificate of Julie A. Demille (201242936RN/201250158NP) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation. She resides in Estacada, Ore.
  • The Board accepted the Voluntary Surrender of Abdulkaliym Ray’s nursing assistant certificate (201402297CNA) for abusing a client and unauthorized removal of property from any person. He resides in Milwaukie, Ore.
  • The Board suspended the nursing assistant certificate of Abbi R. Turner (201011502CNA) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation. She resides in Milwaukie, Ore.
Clatsop County
  • The Board accepted the Voluntary Surrender of Mark Degataga’s nursing assistant certificate (200412360CNA) for performing authorized duties when unable to perform due to a psychological impairment. He resides in Warrenton, Ore.
  • The Board placed the registered nurse license of Jessica R. A. Robbins (201390182RN) on probation for 24 months for inaccurate and incomplete recordkeeping, failing to take action to preserve client safety based on nursing assessment and judgement, and failing to conform to the essential standards of acceptable nursing practice. She resides in Cannon Beach, Ore.
Coos Bay County
  • The Board revoked the registered nurse license of Joseph A. Gonsalves (200942327RN), of Coos Bay, Ore., for violating the terms and conditions of a Board Order.
Hood River County
  • The Board placed the practical nurse license of Julie R. Davis (201609325LPN) on probation for 24 months for attempting to obtain unauthorized prescription drugs and a conviction for Identity Theft. She resides in Mount Hood Parkdale, Ore.
Jackson County
  • The Board placed the nursing assistant certificate of Evan P. Mikkelson (201609353CNA) on probation for 24 months for using intoxicants to the extent or in a manner injurious to himself or others. He resides in Talent, Ore.
Josephine County
  • The Board accepted the Voluntary Surrender of Natalie L. Arbuckle’s nursing assistant certificate (200712427CNA) for a mental impairment. She resides in Grants Pass, Ore.
Lane County
  • The Board placed the nursing assistant certificate of Luis Arciga (201212086CNA) on probation for 24 months for using intoxicants to the extent or in a manner injurious to himself or others. He resides in Springfield, Ore.
  • The Board reprimanded the registered nurse license of Lee A. Cook (200140809RN), of Springfield, Ore., for inaccurate and incomplete recordkeeping, failing to take action to promote client safety, and failing to conform to the essential standards of acceptable nursing practice.
  • The Board reprimanded the nursing assistant certificate of Alanna C. Gardner (000039254CNA), of Eugene, Ore., for abusing a person, and failing to respect client dignity.
  • The Board placed the nursing assistant certificate of Kristen M. Wassom (200910900CNA) on probation for 24 months for using intoxicants to the extent or in a manner injurious to herself or others. She resides in Eugene, Ore.
Marion County
  • The Board accepted the withdrawal of Bonnie Bundy’s RN application for conduct related to impaired function. She resides in Salem, Ore.
  • The Board accepted the Voluntary Surrender of Martin L. Johnson’s registered nurse license (201404926RN) for failing to comply with the terms and conditions of the Health Professionals’ Services Program. He resides in Salem, Ore.
  • The Board accepted the Voluntary Surrender of Claudene L. Waters’s registered nurse license (088000442RN) for abusing a client and failing to respect client dignity. She resides in Salem, Ore.
Multnomah County
  • The Board placed the registered nurse license of Helena H. J. Son (201403863RN) on probation for 12 months for performing new nursing techniques without documented education specific to the technique and clinical preceptored experience to establish competency, and failing to conform to the essential standards of acceptable nursing practice. She resides in Portland, Ore.
Polk County
  • The Board placed the registered nurse license of Christine L. Davis (098007334RN) on probation for 24 months for failing to comply with the terms and conditions of the Health Professionals’ Services Program. She resides in Dallas, Ore.
  • The Board reprimanded the registered nurse license of Titus P. Madlangbayan (201242585RN), of Salem, Ore., for incomplete recordkeeping, failing to take action to promote client safety, and failing to conform to the essential standards of acceptable nursing practice.
Wasco County
  • The Board suspended the nursing assistant certificate of Tiffany M. Logue (201394238CNA) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation. She resides in The Dalles, Ore.

Washington County

  • The Board imposed a $2,500 civil penalty against Kathy A. Poteraj’s registered nurse license (087003038RN) for practicing nursing without a current license. She resides in Tualatin, Ore.
Out of State
  • Arkansas— The Board revoked the registered nurse license of Tiffany P. Gwatney (201508406RN), of Bryant, Ark., for unauthorized removal of drugs from the workplace and using intoxicants to the extent injurious to herself or others.
  • Idaho—The Board accepted the withdrawal of Kaleb M. Contreras’s RN application for conduct related to impairment. He resides in Boise, Idaho.
  • Louisiana—The Board suspended the registered nurse license of Misty D. Mathews (201508285RN) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation. She resides in Boyce, La.
  • Missouri—The Board suspended the registered nurse license of Carrie L. Carter (201502958RN) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation. She resides in Pleasant Hill, Mo.
  • Washington—The Board accepted the withdrawal of Rachel S. Davis’s RN application (200743181RN) for misrepresentation during the licensure process and failing to answer questions truthfully. She resides in Vancouver, Wash.
  • Washington—The Board accepted the Voluntary Surrender of Jade E. Brannan’s registered nurse license (200342113RN) for possessing unauthorized prescription drugs, failing to answer questions truthfully, and failing to conform to the essential standards of acceptable nursing practice. She resides in Washougal, Wash.
