Board of Nursing Disciplinary Actions Taken in October 2012

Clackamas County
  • The Board accepted the voluntary surrender of Peggy S. Casebeer’s Registered Nurse license (078033394RN) for violating the terms and conditions of a Board Order.  She resides in Wilsonville, Ore.
  • The Board reprimanded Betty A. Spaulding’s Practical Nurse license (075008451LPN) for failing to take action to promote client safety, failing to supervise persons to whom nursing tasks have been assigned, and failing to communicate information regarding client status.  She resides in Damascus, Ore.
  • The Board reprimanded Patti E. Dryden’s Registered Nurse license (000033259RN) for implementing standards of nursing care that jeopardize patient safety, and failing to conform to the essential standards of acceptable and prevailing nursing practice.  She resides in Portland, Ore.
  • The Board placed Alicia C. Yeager’s Registered Nurse license (201243380RN) on probation for 24-months due to a DUII conviction.  She resides in Oregon City, Ore.
Clatsop County
  • The Board reprimanded Patricia A. Boullie’s Registered Nurse license and Nurse Midwife Nurse Practitioner certificate (096007245RN/096007245N5) for using the nurse client relationship to exploit the family for personal gain, and incomplete recordkeeping.  She resides in Astoria, Ore.
Josephine County
  • The Board suspended Linda J. Pattinson’s Registered Nurse license (200441423RN) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  She resides in Grants Pass, Ore.
Lane County
  • The Board revoked the nursing assistant certificate of Sunny J. Wescott (201012932CNA), of Florence, Ore., for failing to comply with the terms and conditions of a Board Order.
  • The Board reprimanded Wendy Westphall’s Registered Nurse license (201143395RN) for failing to answer questions truthfully.  She resides in Eugene, Ore.
Linn County
  • The Board revoked the nursing assistant certificate of Coleen M. Peace (200811312CNA), of Albany, Ore., for failing to comply with the terms and conditions of a Board Order.
Marion County
  • The Board suspended Bradford F. Gilman’s Registered Nurse license (088006254RN) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  He resides in Salem, Ore.
  • The Board revoked the Registered Nurse license of Donovan H. Schmidt (200341155RN), of Salem, Ore., for failing to comply with the terms and conditions of the Health Professionals’ Services Program.
  • The Board revoked the Registered Nurse license of James D. Thompson (200842674RN), of Salem, Ore., due to a First Degree Sexual Assault conviction.
Multnomah County
  • The Board suspended Elsa Renthal’s Registered Nurse license (200341753RN) for 30 days with conditions, for client neglect, incomplete recordkeeping, failing to maintain timely client records, and failing to conform to the essential standards of nursing practice.  She resides in Portland, Ore.
  • The Board placed Erick O. Murillo-Camacho’s nursing assistant certificate (201012720CNA) on probation for 24-months for using intoxicants to the extent injurious to himself or others.  He resides in Portland, Ore.
Wasco County
  • The Board accepted the voluntary surrender of Brenda L. Westenskow’s Registered Nurse license (000035411RN) for violating the terms and conditions of the Health Professionals’ Services Program.  She resides in The Dalles, Ore.
  • The Board revoked the nursing assistant and medication aide certificates of Nicole R. Pando (200610385CNA/200620141CMA), of The Dalles, Ore., for failing to respect client rights and dignity, exploiting the client for personal gain, altering medication administration record, and diverting drugs for use by self or others.
Washington County
  • The Board revoked the nursing assistant certificate of Sita V. Levu (000041066CNA), of Hillsboro, Ore., for having a mental condition that prevents her from safely performing CNA duties.
  • The Board revoked the Practical Nurse license of Michelle Evilsizer (200230297LPN), of Beaverton, Ore., for unauthorized removal of drugs from the workplace, and failing to cooperate with the Board during an investigation.
  • The Board placed Alisa R. Walbridge’s Registered Nurse license (099000382RN) on probation for 24-months for attempting to obtain unauthorized drugs, and unauthorized removal of supplies from the workplace.  She resides in Portland, Ore.
  • The Board placed Hope B. Thomas’ Registered Nurse license (200843268RN) on probation for 24-months for failing to take action to preserve client safety, incomplete recordkeeping, and failing to conform to the essential standards of acceptable and prevailing nursing practice.  She resides in Weiser, Ore.
Yamhill County
  • The Board revoked the nursing assistant certificate of Nicholas W. Filer (201210832CNA), of McMinnville, Ore., for failing to comply with the terms and conditions of a Board Order.
  • The Board denied the RN application of Ruth Lazarides (078041321RN)of McMinnville, Ore., for failing to maintain professional boundaries with a client, performing acts beyond her authorized scope, and failing to conform to the essential standards of acceptable and prevailing nursing practice.
Out of State
  • Arizona—The Board revoked the Registered Nurse license of Christopher J. Allen (200541424RN), of Tempe, Ariz., for inaccurate and incomplete recordkeeping, practicing while impaired, and failing to answer questions truthfully.
  • Washington—The Board suspended Robert J. Rodarte’s Registered Nurse license (201140787RN) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  He resides in Tacoma, Wash.
  • Washington—The Board placed Jade E. Brannan’s Registered Nurse license (200342113RN) on probation for 24-months for failing to comply with the terms and conditions of the HPSP.  She resides in Washougal, Wash.
  • Washington—The Board accepted the voluntary surrender of Beverly R. Cooke’s Registered Nurse license (096007546RN) for using intoxicants to the extent injurious to herself or others.  She resides in Vancouver, Wash.
