Board of Nursing Disciplinary Actions Taken in October 2016
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 15 nurses or nursing assistants during its October 12, 2016, Board Meeting.

Clackamas County
  • The Board accepted the Voluntary Surrender of Linda S. Forrest’s practical nurse license (096005028LPN) for abusing a client and failing to take action to preserve client safety. She resides in Milwaukie, Ore.
Columbia County
  • The Board revoked the nursing assistant certificate of Chelsea R. Forbes (201502257CNA), of Saint Helens, Ore., for neglect of a person, documenting the provision of services that were not provided, and failing to answer questions truthfully.
Josephine County
  • The Board placed the practical nurse license of Patricia A. Miller-Rocha (201509432LPN) on probation for 12 months for performing nursing procedures without documented education specific to the procedure, and failing to conform to the essential standards of acceptable nursing practice. She resides in Grants Pass, Ore.
Lane County
  • The Board suspended the registered nurse license of Glen E. Eidson (201340139RN) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation. He resides in Eugene, Ore.
Marion County
  • The Board denied the LPN application of Lilia Mikhailichenko, of Salem, Ore., for failing to answer questions truthfully and failing to cooperate during the course of an investigation.
  • The Board placed the registered nurse license of Steven K. Miller (200942373RN) on probation for 12 months for conduct derogatory to the standards of nursing practice. He resides in Salem, Ore.
  • The Board reprimanded the registered nurse license of Loveth C. Odoemelam (201504952RN), of Salem, Ore., for failing to take action to promote client safety and failing to conform to the essential standards of acceptable nursing practice.
  • The Board placed the nursing assistant certificate of Sierra R. Ortega (201310645CNA) on probation for 24 months for using intoxicants to the extent or in a manner injurious to herself or others, and failing to answer questions truthfully. She resides in Salem, Ore.
Tillamook County
  • The Board accepted the Voluntary Surrender of Suzann L. Repass’s nursing assistant certificate (000027341CNA) for neglecting a person and failing to conform to the essential standards of acceptable nursing assistant performance. She resides in Nehalem, Ore.
Umatilla County
  • The Board denied the LPN application of Amy B. Bray (200530277LPN) for violating the terms and conditions of a Board Order. She resides in Hermiston, Ore.
Washington County
  • The Board accepted the Voluntary Surrender of Primavera A. Cruz-Hamilton’s nursing assistant certificate (200112728CNA) for neglecting a person and jeopardizing the safety of a person in her care. She resides in Beaverton, Ore.
  • The Board placed the registered nurse license of Mollie D. McAmis (201504192RN) on probation for 12 months for inaccurate recordkeeping, and falsifying a client record. She resides in Hillsboro, Ore.
Out of State
  • Florida—The Board accepted the Voluntary Surrender of Lois J. McGinnis’s registered nurse license (201503722RN) for incomplete and inaccurate recordkeeping, and the unauthorized removal of drugs from the workplace. She resides in Leesburg, Fla.
  • Texas—The Board accepted the withdrawal of Ruthann N. Hoagland’s RN application for conduct derogatory to the standards of nursing. She resides in Caldwell, Texas.
  • Washington—The Board reprimanded the practical nurse license of Francisco M. Lara (200230443LPN), of Gig Harbor, Wash., for failing to implement the plan of care and failing to conform to the essential standards of acceptable nursing practice.
