Clackamas County
  • The Board reprimanded Richard W. Smith’s Registered Nurse license (200242224RN) for inaccurate and incomplete recordkeeping, and failing to maintain client records in a timely manner.  He resides in West Linn, Ore.
  • The Board denied the CNA application of Thomas R. Hermansen of Portland, Ore., for using intoxicants to the extent injurious to himself or others, and failing to provide documents requested by the Board.

Coos County

  • The Board placed Joshua A. Jennings’ nursing assistant certificate (200910518CNA) on probation for 24-months for willful misrepresentation during the certification process, and failing to cooperate with the Board during an investigation.  He resides in North Bend, Ore.
  • The Board suspended Donna D. Willson’s Registered Nurse license (091006861RN) for 60 days, for client neglect, incomplete recordkeeping, failing to take action to promote client safety, and failing to conform to the essential standards of nursing practice.  She resides in Bandon, Ore.

Crook County

  • The Board revoked the nursing assistant certificate of Kirsten L. Vantzelfden (200811572CNA), of Prineville, Ore., for using intoxicants to the extent injurious to herself or others.

Douglas County

  • The Board revoked the nursing assistant certificate of Kayla D. Snyder (200810750CNA), of Roseburg, Ore., for incomplete recordkeeping, failing to competently perform CNA duties, attempting to obtain unauthorized drugs, and using intoxicants to the extent injurious to herself or others.
  • The Board reprimanded Corinne A. Alvarez’s practical nurse license (201030437LPN) for failing to maintain professional boundaries with a patient, engaging in sexual contact with a client, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Drain, Ore.

Jackson County

  • The Board accepted the voluntary surrender of Paul A. Barry’s nursing assistant certificate (000036392CNA) for violating the terms and conditions of a Board Order.  He resides in Rogue River, Ore.
  • The Board suspended Paige M. Garcia’s nursing assistant certificate (200712283CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  She resides in Medford, Ore.

Josephine County

  • The Board placed Raymond M. Belsheim’s nursing assistant certificate (200810522CNA) on probation for 24-months for using intoxicants to the extent injurious to himself or others.  He resides in Cave Junction, Ore.

Lane County

  • The Board accepted the voluntary surrender of Hugo S. Okonogi’s Registered Nurse license (200340540RN) for violating the terms and conditions of a Board Order.  He resides in Eugene, Ore.
  • The Board placed Nicole E. Kelley’s nursing assistant certificate (201112450CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Florence, Ore.
  • The Board suspended Greg A. Harrison’s nursing assistant certificate (000025259CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  He resides in Cottage Grove, Ore.
  • The Board revoked the nursing assistant certificate of Philip E. Atkisson (000005244CNA), of Springfield, Ore., for failing to answer questions truthfully and failing to provide documents requested by the Board.
  • The Board revoked the nursing assistant certificate of Leo J. Burns (200110755CNA), of Florence, Ore., for failing to cooperate with the Board during an investigation.
  • The Board denied the RN application of Stuart C. Moore (097006123RN) of Eugene, Ore., for failing to provide sufficient documentation of reformation of the issues that originally brought the licensee to the Board’s attention.

Linn County

  • The Board suspended Ruth J. Merrill’s Registered Nurse license (076037170RN) for 30-days, to be followed by 24-months of probation for failing to take action to preserve client safety, assuming duties without documented preparation for the duties, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Lebanon, Ore.

Marion County

  • The Board placed Natausha M. Glover’s nursing assistant certificate (000038339CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Salem, Ore.
  • The Board placed Cynthia L. Chiado’s nursing assistant certificate (200412368CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Salem, Ore.
  • The Board suspended James A. Bathke’s nursing assistant certificate (201012865CNA) for a minimum of 14 days, for failing to cooperate with the Board during an investigation.  He resides in Stayton, Ore.
  • The Board denied the CNA application of Roberto Garcia of Salem, Ore., for failing to answer questions truthfully during the certification process and failing to cooperate during an investigation.

Multnomah County

  • The Board accepted the voluntary surrender of Virginia L. Tan’s Registered Nurse license (200942916RN) for violating the terms and conditions of a Board Order.  She resides in Portland, Ore.
  • The Board reprimanded Anca Nickelson’s nursing assistant certificate (201012945CNA) for failing to competently perform CNA duties.  She resides in Portland, Ore.

Polk County

  • The Board reprimanded Sandra D. Levy’s practical nurse license (088005219LPN) for violating patient rights to privacy, unauthorized removal of patient records, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Salem, Ore.
  • The Board placed Laura J. Guffey’s Registered Nurse license (095000531RN) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Dallas, Ore.

Washington County

  • The Board placed Carey A. Walters’ Registered Nurse license (200141096RN) on probation for 24-months for unauthorized removal of narcotics from the workplace, and using intoxicants to the extent injurious to herself or others.  She resides in Beaverton, Ore.
  • The Board accepted the voluntary surrender of Doris P. Vaughn’s nursing assistant certificate (200612662CNA) for client neglect and failing to competently perform CNA duties.  She resides in Beaverton, Ore.
  • The Board revoked the Registered Nurse license of Kami M. Jackson (200442050RN), of Beaverton, Ore., for leaving a nursing assignment, incomplete recordkeeping, practicing nursing while impaired, and failing to provide requested documents by the Board.
  • The Board reprimanded Carolynn C. Perciballi-Taub’s Registered Nurse license (200742486RN) for inaccurate recordkeeping, falsifying a client record, and improperly delegating tasks of nursing care to unlicensed persons.  She resides in Hillsboro, Ore.

Yamhill County

  • The Board revoked the nursing assistant certificate of Alana L. Lenahan (000039760CNA), of Newberg, Ore., for using intoxicants to the extent injurious to herself or others, and failing to cooperate with the Board during an investigation.

Out of State

  • Washington—The Board denied the CNA application of Mindy Schilling of Camas, Wash., for failing to answer questions truthfully during the certification process.
  • West Virginia—The Board denied the LPN application of Karen F. Knight (200530088LPN) of McMechen, W.V., for misrepresentation during the licensure process and failing to answer questions truthfully.
  • Arizona—The Board ordered the emergency suspension of Christopher J. Allen’s Registered Nurse license (200541424RN) for practicing nursing while physical and mentally impaired, and failing to answer questions truthfully.  He resides in Tempe, Ariz.
  • New Mexico—The Board reprimanded Karla J. Williams’ Registered Nurse license (201142880RN) for practicing nursing without a current Oregon license.  She resides in Santa Fe, N.M.
