Board of Nursing Disciplinary Actions Taken in September 2013

Clackamas County
  • The Board accepted the voluntary surrender of Aleksandr A. Voloshin’s nursing assistant certificate (200410930CNA) for violating a Board Order.  He resides in Damascus, Ore.
  • The Board revoked the Registered Nurse license of Sadie E. Hanks (200242532RN), of Milwaukie, Ore., for failing to respect client dignity, client abuse and neglect, inaccurate and incomplete recordkeeping, and failing to conform to the essential standards of acceptable nursing practice.
Clatsop County
  • The Board suspended Stacey I. Scruggs’s Registered Nurse license (077039062RN) for 60 days for failing to implement the plan of care, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Warrenton, Ore.
Coos County
  • The Board suspended Barbara J. McCabe’s practical nurse license (082020666LPN) for 60 days for failing to respect client dignity, threatening behavior towards a coworker in the presence of a client, and engaging in other unacceptable behavior towards clients.  She resides in Bandon, Ore.
  • The Board revoked the nursing assistant certificate of Samuel G. Carbaugh (200313581CNA), of Coos Bay, Ore., for client neglect, and failing to perform CNA duties competently.

Deschutes County

  • The Board suspended Jena D. Christiansen’s Registered Nurse license (200742215RN) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Bend, Ore.
Douglas County
  • The Board reprimanded Peggy L. Meharry’s Registered Nurse license (090003224RN) for failing to maintain professional boundaries with a client.  She resides in Roseburg, Ore.
  • The Board suspended Renae C. Green’ nursing assistant certificate (200611824CNA) for a minimum of 14 days for failing to cooperate with the Board during an investigation.  She resides in Sutherlin, Ore.
Jackson County
  • The Board placed Deborah J. Roper’s practical nurse license (084052253LPN) on probation for 24-months for client abuse and neglect, failing to take action to promote client safety, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Medford, Ore.
  • The Board reprimanded Janet N. Lajoie’s Registered Nurse license (099000598RN) with conditions for client neglect, incomplete recordkeeping, failing to follow the plan of care, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Rogue River, Ore.
Lane County
  • The Board placed Susan J. Allen’s Registered Nurse license (200241604RN) on probation for 24-months for failing to conform to the essential standards of acceptable nursing practice.  She resides in Eugene, Ore.
  • The Board placed Aaron R. Boyce’s practical nurse license (200130150LPN) on probation for 12-months due to impairment.  He resides in Eugene, Ore.
    Lincoln County
  • The Board accepted the voluntary surrender of Jolona K. Cantrell’s nursing assistant certificate (200711651CNA) for client neglect, and using intoxicants to the extent injurious to herself or others.  She resides in Newport, Ore.
Marion County
  • The Board reprimanded Jessica A. Mosbrucker’s nursing assistant certificate (000035399CNA) for failing to perform CNA duties competently.  She resides in Salem, Ore.
  • The Board accepted the voluntary surrender of Linda M. Lemmon’s Registered Nurse license (090000439RN) for failing to conform to the essential standards of acceptable nursing practice, and violating the terms and conditions of the Nurse Monitoring Program.  She resides in Woodburn, Ore.
Multnomah County
  • The Board placed Amy P. Simpson’s nursing assistant certificate (201392955CNA) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Portland, Ore.
  • The Board revoked the Registered Nurse license of Paige E. Dasaro (099006757RN), of Portland, Ore., for failing to comply with the terms and conditions of the Health Professionals’ Services Program.
  • The Board placed Alyssa A. Aurisy’s Registered Nurse license (200741244RN) on probation for 36-months for the unauthorized removal of drugs from the workplace, and using intoxicants to the extent injurious to herself or others.  She resides in Portland, Ore.
Polk County
  • The Board reprimanded Keith D. Gunter’s Registered Nurse license (201141989RN) with conditions for client abuse.  He resides in Salem, Ore.
Washington County
  • The Board placed Jamie M. Olinger’s Registered Nurse license (200542111RN) on probation for 24-months for using intoxicants to the extent injurious to herself or others.  She resides in Cornelius, Ore.
  • The Board placed Cheryl L. Pippin’s Registered Nurse license and Pediatric Nurse Practitioner certificate (099006263RN/099006263N2) on probation for 18-months for inaccurate and incomplete recordkeeping, failure to properly assess and document client assessment, and failing to communicate information regarding client status to the health care team.  She resides in Beaverton, Ore.
  • The Board placed Connie L. Miyao’s Registered Nurse license (201042720RN) on probation for 24-months for violating the terms and conditions of the Health Professionals’ Services Program.  She resides in Sherwood, Ore.
  • The Board accepted the voluntary surrender of Jeffrey N. McAllister’s Registered Nurse license (200841729RN) due to allegations that he sexually assaulted multiple female patients.  He resides in Portland, Ore.
Yamhill County
  • The Board placed Beth A. Roessel’s Limited Registered Nurse license (L201392879RN) on probation for 24-months for violating the terms of the Nurse Monitoring Program.  She resides in Newberg, Ore.
  • The Board reprimanded Holly A. Mitchell’s nursing assistant certificate (000009931CNA) for client abuse.  She resides in McMinnville, Ore.
  • The Board placed Susan J. Messner’s Registered Nurse license (201042644RN) on probation for 24-months for unauthorized removal of drugs from the workplace, and using intoxicants to the extent injurious to herself or others.  She resides in McMinnville, Ore.
Out of State
  • California—The Board accepted the voluntary surrender of Brent E. Egemo’s Registered Nurse license and Family Nurse Practitioner certificate (200942485RN/200950132NP) due to discipline in another state.  He resides in Lancaster, Calif.
  • Massachusetts—The Board denied the RN application of Anne E. Morrison, of Groton, Mass., for failing to answer questions truthfully during the licensure process, and misrepresentation in applying for a license.
  • Nevada—The Board denied the LPN application of Michelle Rose G. Lingad, of Sparks, Nevada, for failing to meet educational requirements for licensure.
  • Washington—The Board placed Robert J. Wattez’s Registered Nurse license (097000117RN) on probation for 24-months for practicing nursing while impaired, and abusive behavior toward coworkers.  He resides in Vancouver, Wash.
