Board of Nursing Disciplinary Actions Taken in September 2017
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 34 nurses or nursing assistants during its September 13, 2017, Board Meeting.

Benton County
  • The Board denied the RN application of Joshua N. Gaffney, of Monroe, Ore., for engaging in unacceptable behavior toward a client, using intoxicants to an extent or in a manner injurious to himself or others, and failing to answer questions truthfully.
Clackamas County
  • The Board denied the CNA application of Juliet M. Jaynes, of Oregon City, Ore., for using intoxicants to the extent or in a manner injurious to herself or others, failing to cooperate with the Board during an investigation, and failing to answer questions truthfully.
  • The Board revoked the registered nurse license of Sharon J. Kent (097000584RN), of Happy Valley, Ore., for incomplete recordkeeping and obtaining unauthorized drugs.
  • The Board accepted the Voluntary Surrender of Nola J. Maloney’s registered nurse license (000031617RN) for the unauthorized removal of property from the workplace or any person.  She resides in Lake Oswego, Ore.
  • The Board accepted the Voluntary Surrender of Theresa A. Vos’s nursing assistant and medication aide certificates (200312935CNA/200520002CMA) for willful misrepresentation during the licensure process, convictions for crimes that bear demonstrable relationship to CNA duties, and entering inaccurate documentation into a health record.  She resides in Boring, Ore.
Coos County
  • The Board reprimanded the registered nurse license of Bobbie M. Felton (200942261RN), of Coos Bay, Ore., for improperly delegating tasks of nursing care to unlicensed persons.
  • The Board suspended the nursing assistant certificate of Rebecca A. Youngman (200313056CNA) for 30 days for failing to maintain professional boundaries.  She resides in North Bend, Ore.
Curry County
  • The Board placed the nursing assistant certificate of Jessica J. James (200212318CNA) on probation for 24 months for using intoxicants to the extent or in a manner injurious to herself or others.  She resides in Port Orford, Ore.
Deschutes County
  • The Board revoked the registered nurse license of Amanda G. Greening (200942136RN), of Bend, Ore., for violating the terms and conditions of a Board Order.
Jackson County
  • The Board reprimanded the practical nurse license of Janice G. Jackson (000007212LPN), of Central Point, Ore., for the unauthorized removal of property from any person.
  • The Board suspended the nursing assistant certificate of Patricia A. Pierce (000035860CNA) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation.  She resides in Phoenix, Ore.
  • The Board placed the registered nurse license of Sage Thompson (200441424RN) on probation for 24 months for using intoxicants to the extent or in a manner injurious to herself or others.  She resides in Phoenix, Ore.
Josephine County
  • The Board denied the CNA application of Samantha M. Cole, of Grants Pass, Ore., for misrepresentation during the licensure process and failing to cooperate with the Board during an investigation.
  • The Board reprimanded the registered nurse license of Paula K. Olson (201407151RN), of Grants Pass, Ore., for incomplete recordkeeping, failing to take action to preserve client safety, and failing to conform to the essential standards of acceptable nursing practice.
Lane County
  • The Board suspended the nursing assistant certificate of Christina A. Cowles (200812215CNA) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation.  She resides in Eugene, Ore.
  • The Board placed the registered nurse license of Stuart C. Moore (097006123RN) on probation for 24 months for the unauthorized removal of drugs from the workplace, and practicing nursing while impaired.  He resides in Eugene, Ore.
  • The Board accepted the Voluntary Surrender of Elizabeth J. Thaxton’s nursing assistant certificate (200712639CNA) for violating a person’s rights to privacy.  She resides in Creswell, Ore.
  • The Board reprimanded the family nurse practitioner certificate of Julie B. Willardson (201404741NP-PP), of Eugene, Ore., for practicing nursing while impaired.
Lincoln County
  • The Board revoked the nursing assistant certificate of Kathleen M. Ford (201507189CNA), of Siletz, Ore., for abusing a person, neglecting a person, and failure to report to the Board a felony arrest.
Linn County
  • The Board placed the nursing assistant certificate of Jane M Kline (201400974CNA) on probation for 24 months for using intoxicants to the extent or in a manner injurious to herself or others.  She resides in Sweet Home, Ore.
Multnomah County
  • The Board accepted the Voluntary Withdrawal of Krystal R. Aprobert’s RN application for using intoxicants to the extent or in a manner injurious to herself or others.  She resides in Portland, Ore.
  • The Board placed the registered nurse license of James J. Body (200742322RN) on probation for 24 months for using intoxicants to the extent or in a manner injurious to himself or others.  He resides in Portland, Ore.
  • The Board reprimanded the practical nurse license of Esmeralda M. Doctolero (201030465LPN), of Portland, Ore., for inaccurate and incomplete recordkeeping, neglecting a client, and failing to conform to the essential standards of acceptable nursing practice.
  • The Board accepted the Voluntary Surrender of Mohini L. Kumar’s nursing assistant certificate (200211163CNA) for demonstrated incidents of dishonesty, misrepresentation, or fraud.  She resides in Portland, Ore.
  • The Board placed the registered nurse license of Michele L. Renninger (201707584RN) on probation for 24 months for using intoxicants to the extent or in a manner injurious to herself or others.  She resides in Portland, Ore.
  • The Board revoked the nursing assistant certificate of Ruth A. Whitaker (200410835CNA), of Portland, Ore., for using intoxicants to the extent or in a manner injurious to herself or others, and failing to provide documents requested by the Board.
Polk County
  • The Board placed the registered nurse license of Leslie M. Broyles (201505277RN) on probation for 24 months for the unauthorized removal of drugs from the workplace, and using intoxicants to the extent or in a manner injurious to herself or others.  She resides in Salem, Ore.
  • The Board suspended the nursing assistant certificate of Leslie A. Warren (000017890CNA) for 30 days for engaging in verbal abuse in the presence of the client.  She resides in Salem, Ore.
Umatilla County
  • The Board accepted the Voluntary Surrender of Meghan K. Bruns’s registered nurse license (201393156RN) for violating the terms and conditions of a Board Order.  She resides in Pendleton, Ore.
Wasco County
  • The Board placed the practical nurse license of Joseph N. Schneider (201404489LPN) on probation for 24 months for incomplete recordkeeping and using intoxicants to the extent or in a manner injurious to himself or others.  He resides in The Dalles, Ore.
Washington County
  • The Board suspended the nursing assistant certificate of Jenifer M. Gaskin (201392602CNA) for a minimum of 14 days for failing to cooperate with the Board during the course of an investigation.  She resides in Tualatin, Ore.
  • The Board denied the CNA application of Bradley R. Tanberg, of Tigard, Ore., for conviction of a crime that bears demonstrable relationship to CNA duties.
Out of State
  • California—The Board placed the registered nurse license of Michele L. Fournier (201707579RN) on probation for 24 months for convictions of crimes that bear a demonstrable relationship to the practice of nursing.  She resides in Fresno, Calif.
  • Washington—The Board placed the registered nurse license of Andrea E. Grant (200642346RN) on probation for 24 months for inaccurate and incomplete recordkeeping, and using intoxicants to the extent or in a manner injurious to herself or others.  She resides in Vancouver, Wash.
