SSNs Requirement
If the U.S. Social Security Administration issued you a Social Security Number (SSN), you are required to provide it on your initial (by either exam or endorsement) nursing license or nursing assistant certificate applications.

If You Don't Have a SSN
If the U.S. Social Security Administration never issued you a Social Security Number (SSN), you may sign a Statement of No Social Security Number form instead. The OSBN will not issue licenses or certificates to licensees who have a SSN and refuse to disclose it, or have never been issued a SSN and refuse to sign the statement.

Why We Collect SSNs
State and federal law requires us to collect licensee SSNs. This record of your SSN will be used for child support enforcement and tax administration purposes (including identification) only, unless you authorize other use. If the OSBN takes any disciplinary action against your license/certificate, we will report your SSN to the federal Health Care Integrity and Protection Data Bank.

How OSBN Safeguards SSNs
The OSBN takes the responsibility to safeguard your personal information very seriously. Your SSN will not be displayed on your license or certificate, or released on OSBN records. Only qualified government agencies are allowed access to your SSN. We routinely shred extraneous records containing personal information.
